COMES NOW, and unto this Honorable Board most respectfully allege:
1. That the movant is of legal age, Filipino and an applicant of an Application for New Certificate of
Public Convenience to operate a TRANSPORTATION NETWORK VEHICLE SERVCE
(TNVS) on the line of METRO MANILA from the said place to any point in Luzon
accessible to motor vehicle traffic and with use of One (1) unit under the above Case Number
which is more particularly described as follows:

2. That applicants would like to request this Honorable Board for the issuance of EXTENSION OF
PROVISIONAL AUTHORITY to use for the TNVS while waiting for the release of Certificate
of Public Convenience.
3. That in support of this Motion for Issuance of Provisional Authority, applicant is submitting the
following documentary requirements for the evaluation of the Honorable Board to wit:

Annex “A” - Application for New CPC

Annex “B” - Official Receipt of Registration (OR)
Annex “C” - Certificate of Registration (CR)
Annex “D” - Government ID

Wherefore in view of the above it is respectfully request that the above Motion
for issuance of Provisional Authority be GRANTED.

Other relief just and equitable under the premises is likewise prayed for.
