2016/043-00 issued to HC CONSUMER FINANCE PHILIPPINES, INC. The insurance cover is subject to the terms, provisions and
limitations of the said policy.

Upon receipt and approval of due proof/s of death of the Insured Loan Borrower, SLGFI shall pay to the Policyholder, HC CONSUMER

later than 180 days from date of death. Failure to submit the written notice and proof of claim within the time limits shall not invalidate or reduce any claim if it
