Main findings of the inspection

H1-Work at height

* Workers carrying out welding job on the scaffold are at the risk of being fall or falling objects from scaffold.
Another high risk is death or serious injuries from collapse of scaffold. Because safe working load of scaffold is 300
kg but weight on platform in nearby 450 kg. There is no one using full body harness. The severity of falling
persons, falling objects or scaffold is very high and it can be lead to several injuries or death.

* Immediate measures like fitting guard rails to the work platform. Platform must not be overloaded. Ensure the
use of Personal Protective Equipment but it would not eliminate or reduce hazard. I recommend that tower must
be sited on firm, level ground. Another platform must be provided to carry extra load. Safety nets or mesh must
be used here to prevent falling objects on nearby peoples. Use of scaffold must be restricted to trained and
authorised staff only. Additionally inspections of scaffold be carried out daily and work area must be barricaded.

* Owing to this observation, the company is failing to comply with the requirements of the Building and other
construction workers (Regulation of employment and condition of service) act 1996 as well as Safety and Health
in Construction Convention 1988 (No. 167) as this condition risk being extreme high to human lives. It is important
to address this as earlier. The immediate cost is nominal (around 25000 Rs) where as the cost of significantly
reduce the hazard permanently is extreme less (around 100000 Rs) in comparison to the losses which the
company may be liable to pay in case of an accident arising out of this hazard.

H6 - Electricity –

* Workers carrying out maintenance jobs on electrical equipment at the risk of electrocution to workers as the
faulty equipments are used. Earthing Leakage Circuit Breakers (ELCB) not provided to incoming power circuits to
the main electrical panel. Use of portable power tools, hand held welding machines and other devices pose a
serious electrocution risk to these workers.

* Immediate measures like replacement of faulty equipments. All incoming power circuits need to be provided
ELCB. Ensure the use of PPE but it would not eliminate or reduce hazard. I recommend that, there must be in-built
ELCB panel and monitored by competent electrical engineer on a weekly basis.
* Owing to this observation, the company is failing to comply with the requirements of The Electricity rules 2003
and The Building and Other Construction workers Act 1996 and Electricity at work regulations -1989 as this
condition severely risk of some human lives. It is very important to solve this issue as earlier as possible. The
immediate cost is nominal (around 30,000 Rs). In Compare to losses which can company may pay in case of an
accident arising out of this hazard.

H2 - Excavation –

* Workers carrying out excavation work are at the risk of serious injuries, death of workers due to being fall or
sides collapse of excavation. There may be additional high risk of toxic gases, oxygen deficiency.
There is one using Respiratory Protective Equipment. The severity of falling persons in excavation, sides collapse
or flooding is very high and it can be lead to several injuries or death.

Immediate measure like place barriers consist of guard rails to prevent people falling in. Gas and other toxic
substances tests must be done before work start. I recommend that, Sides of excavations should be supported
with shoring and trench box should be used. Additionally Respiratory Protective Equipment must be provided.
Excavation should be inspected before the start of every work shift.

Owing to this observation the company is failing to fulfil the requirement of building and Other Construction
Workers Act 1996 as well as Safety and Health in Construction Convention 1988 (no. 167) as this condition risk
being extreme high to human lives. It is important to address this as earlier. The immediate cost is nominal
(around 50000 Rs) where as the cost is significantly reduce this hazard permanently is extreme less (around
100000 Rs) in comparison to the losses which the company may be liable to pay in case an accident arising out of
this hazard.

H7 - Fire

In the company canteen kitchen chef making food and additional gas cylinders and fuels kept beside kitchen are at
the risk of possible fire, serious burns or accidents. There may be additional high risk of explosion from stored
fuel. There is no adequate ventilation for kitchen and improper housekeeping. The severity of fire accidents is very
high and it can be lead to several injuries, death or property damage as well as possible civil claims and
imprisonment.

Immediate measures like fuel storage in separated, well ventilated area. Adequate fire extinguishers must be
installed. I recommend that good ventilation is must provided for kitchen room. Ignition sources are to be
removed from fuel storage; electrical appliances in kitchen should be inspected tested in relation with fire
resistance. Additionally fire proof doors provided for these area and kitchen staff and other workers provide
training about deal with Fire Emergencies.

Owing to this observation the company is failing to comply with the requirements of fire protection and
prevention act 1997 and Factories act 1948 as this condition risk being extreme high to human lives. It is very
important to address this as soon as possible. The immediate cost is nominal (around 50000 Rs) where as the cost
to significantly reduce this hazard permanently is extreme is less (around 300000 Rs) in comparison to the losses
which the company may be liable to pay in case of accident occurrence out of this hazard.

H4 – Demolition

Workers carrying out demolition work of a old building structure in company premises are at the risk of possible
injuries, death or serious injuries. There may be an additional high risk of premature collapse of structure and
people got injured or possible deaths. There is no barricading for other workers and vehicles no one presenting
there as a banks men. The severity of possible fatalities and injuries is very high and it can result to major
accidents.
Immediate measures like barricading the area where demolition work are going damping down water spray to
reduce dust from demolition. I recommend that the site must be secured with fencing or hoardings, any
hazardous materials removed and disposed from previous work. Additionally long reach machine must be used
and equipment must be inspected and maintained periodically as per norms.

Owing to this observation company is failing to mitigate this hazard does not comply with the provisions of the
building and other construction workers act 1996. The cost of mitigating this hazard is extreme loss (around
150000 Rs) in comparison to the losses which the company may be liable to pay in case of an accident arise out of
this hazard.
