YEAR-END ACCOMPLISHMENT REPORT


January 2013– December 2013

I. INTRODUCTION:

Human precisely welcomes anything that is consigned with


accomplishment because success has always been the aspiration of man.
Through this, man learns, tends to adhere, and makes more out of the good
things that he receives everyday. A person usually embraces the more beneficial
results of his toil and feels good for great accomplishment.
This year-end accomplishment report includes data on expanding the
access, quality, and governance of basic secondary education. Performance
indicators, promoting and strengthening of local cultures and arts, physical
fitness and sports program, rationalizing the management of scarce resources,
promoting welfare of the staff, and implementation of special programs and
projects are the basic contents that pacify the actualization of every objective
and target.

II. OBJECTIVES AND TARGETS.

1. To promote quality education


2. To make an account to the access to basic education
3. To assist all major activities from June 2013 to March 2014

III. ACCOMPLISHMENT:

A. ACCESS

The school is under the supervision of the District Supervisor. Also, a


Division Monitoring Supervisor is assigned to look into the operations of the
school. There are 74 Secondary teachers working hand in hand with 2 Head
Teacher-III, and 1 Secondary School Principal.

1. Enrolment

As seen in Figure 1, there is an increasing trend of the yearly enrolment of


Sogod National High School. For the SY: 2011-2012, the total enrolment was
2,188 which was composed of 1,110 male and 1,078 female. In the SY: 2012-
2013, it increases into 2,352 which was composed of 1,170 male and 1,182
female which means that an increase of 164 students had taken place. For the
SY: 2013-2014, the enrolment increases to 2,593 which is composed of 1,296
male and 1,297 female.

Table 1 shows the Health and Nutritional status of students in the school year 2013-
2014. It is then reflected that most of the ailments of students from Grade-7 to Fourth Year
is Tenia Flava (208). It is followed by dental problems as reflected in the number of
students who received the dental care service. Cough also constitutes as one of the
common ailments treated with the total number of 117 cases; common colds follows with
96 recorded cases; minor injuries were also reported which constituted 44 cases. There
were also recorded cases of pale conjunctiva (34), skin allergy (16), fever (6), headache (1),
and defective vision (1). Generally, there were 115 students whose nutritional health status
was wasted and 31 others have the nutritional health status of severely wasted.

B. QUALITY

Figure 2 shows the academic accomplishment of Sogod National High School in


terms of the National Achievement Test in the School Year 2012-2013. As seen in the
figure, Math has the highest MPS which is 77.03 and it is followed by Araling Panlipunan
which is 75.34. This means that only Math and Araling Panlipunan exceed the national
target MPS of 75%. Also shown in the figure is the result of English which is 69.55. It is
followed by Filipino which MPS is 68.05. Furthermore, Science got the MPS of 48.98 and
Critical Thinking has the lowest MPS of 43.29. In the average, the MPS of Sogod National
High School in the National Achievement Test for the School Year 2012-2013 is 65.93.
2. Passers of Accreditation and Equivalency Test (A and E)

For the School Year 2013-2014, the Open High School (OHS) program of Sogod
National High School has the total enrolment of 38. Among them, 6 had completed the
requirements for graduation and successfully marched during the Commencement
Exercises last March 28, 2014. And, in the recent examination of the Accreditation and
equivalency (A and E) test, 10 among the OHS students passed. This result is presented in
Figure 3.

3. Passers of DOST, UPCAT, PMMA, and UC (2014)

Figure 4 shows the number of passers of Entrance Test and Scholarship Grant. For
the School Year 2013-2014, there were 10 graduating students under the STE (Science
Technology and Engineering) program who took the University of the Philippines College
Admission Test (UPCAT). And, from them, 7 had passed. There were 6 students under the
same program who took the scholarship grant of the Department of Science and
Technology (DOST). And, 2 of the examinees passed. Other institutions that offer
scholarship grants to some of the Fourth Year students of Sogod National High School were
the Philippine Marine Maritime Academy and the University of Cebu Norwegian
Scholarship. Figure 4 further reflects that among the 6 examinees of PMMA, 2 had passed
which makes the percentage of passing into 30%. Moreover, it can be gleaned in Figure 4
that there were 4 passers out of 8 examinees of the University of Cebu Norwegian
Scholarship which makes the percentage of passing into 50%.

4. Increasing Graduation Rate (SY: 2013-2014)

The Graduation rate of Sogod National High School for the last three School Years is
shown in Figure 5. An seen, an increasing trend is taking place hence the baseline was only
92.63 (male) and 93.51 (female) for the SY: 2011-2012; and for the SY: 2013-2014, the
male is 99.77 and the female is 100.00.

5. Decreasing Dropout Rate (SY: 2013-2014)

Figure 6 shows the Dropout Rate of Sogod National High School for the last three
School Years. It is seen in this figure that a fluctuating trend is taking place hence the
baseline (SY: 2011-2012) says that the male dropout is 0.88 and it rises to 1.48 in SY: 2012-
2013 and reduces again to 1.08 in the SY: 2013-2014. The female on the other hand, is
showing a decreasing trend hence the baseline (SY: 2011-2012) is 1.4 and lowers down to
0.94 in SY: 2012-2013 and lowers down again to 0.23 in SY: 2013-2014.

6. Decreasing Failure Rate

Figure 7 shows the Failure of Sogod National High School for the period of 3 school
years. As seen in the figure, a fluctuating trend is taking place. As evident, in SY: 2011-2012,
the male failure rate is 11.15 while the female is 4.85. In SY: 2012-2013, the rate of the
male increases to 11.28 while the female continuously decreases to 2.5. And, in SY: 2013-
2014, the failure of the male is only 2.31 while the female is 0.77.

7. Decreasing Repetition Rate


Figure 8 shows a fluctuating repetition Rate for the last three School Years. It is seen
in this figure that SY: 2011-2012 has the male repetition rate of 4.81 and female repetition
rate of 0.19. In SY: 2012-2013, the rate increases hence the male is 12.46 and female is
3.77. And, the SY: 2013-2014 shows a decrease of rate hence the male has 0.9 and the
female has 0.8.

8. Increasing of Completion Rate

Figure 9 shows an increasing Completion Rate for the last three School Years. It is
seen in this figure that SY: 2011-2012 has the male Completion Rate of 74.22 and the is
74.83. In SY: 2012-2013, the rate of the male decreases to 67.87 while the female rate is
90.53. And, the SY: 2013-2014 shows again an increasing trend of rate hence the male rises
to 80.25 and the female rises to 95.95.

9. Increase of Promotion Rate

Figure 10 shows an increasing trend of Promotion Rate for the last three School
Years. It is seen in this figure that SY: 2011-2012 has the male Promotion Rate of 80.25 and
the female is 86.23. In SY: 2012-2013, the rate of the male increases to 89.53 and the
female rate increases to 97.53. And, the SY: 2013-2014 shows again an increasing rate
hence the male rises to 97% and the female rises to 99.04.

10. Awards, Citations and Winnings (SY: 2013-2014)

In order to promote student development, every learning client of Sogod National


High School is encouraged to participate in every academic, cultural and sports
competitions and other scholastic activities in different areas and levels. As evidence to
this, below are the records of awards, citations, and winnings which Sogod National High
School had garnered from June 2013 to March 2014.

In MATHEMATICS, the following were won:


1. First Place in Math Grade 7- Quiz BEE (District Level)
2. First Place in Math Grade 8- Quiz BEE (District Level)
3. First Place in Math 3rd Year- Quiz BEE (District Level)
4. First Place in Math 4th Year- Quiz BEE (District Level)
5. First Place in Math Grade 7- Quiz BEE (IAD Level)
6. First Place in Math Grade 8- Quiz BEE (IAD Level)
7. First Place in Math 3rd Year- Quiz BEE (IAD Level)
8. First Place in Math 4th Year- Quiz BEE (IAD Level)

5. 1st Place in Science Jingle (Regional Level)
6. 2nd Place in Bb. Makakalikasan (Regional Level)
7. 1st Place in Science Quiz Bee (POSTE-Regional Level)

In English, the following were won:


1. 1st Place in Declamation Contest (Division Level)
2. 2nd Place in Spelling Bee (IAD Level)
3. 2nd Place in Spelling Bee (Division Level)
4. 2nd Place in Oration Contest (IAD Level)
In the SPECIAL PROGRAM IN SPORTS (SPS), the following were achieved:
1. 2nd Place in Chess Competition (Boys Category) in Area Meet
2. 2nd Place in Chess Competition (Girls Category) in Area Meet
3. Champion in Basketball (Provincial Meet)
4. 1st Place in Batang Pinoy Boxing (Regional Level)
5. 1st Place in Takraw Batang Pinoy (Regional Level)
6. 2nd Place in Lawn Tennis Batang Pinoy (Regional Level)

In the SPECIAL PROGRAM IN ARTS (SPA), the following were achieved:

1. Best Performing Group for Dance Drama (National Conference Workshop)


2. Best Performing Group for Dance “Sulog” during Sogod Town Fiesta Celebration
held on Dec. 14-15, 2013

In the Technology and Livelihood Education (TLE), the following were


achieved:
1. 1st Place in Tarpaulin Design (Technolympics Exhibition 2013)-District Level

11. Reading Literacy Rate of Students

Reading is a powerful tool of man that is used in order to live and survive (Ramos,
2011. By this byline, Sogod National High School kept on tracking the student reading
literacy rate of the 7 and 8 graders both in English and Filipino. The students were tested in
their silent and oral reading level using the Phil. IRI reading instruments. All the results are
presented in Tables 2a, 2b, 2c, and 2d.

12. Different Strategies Applied in order to Achieve the above


Accomplishments:

12.1. Closely monitoring of classroom teaching-learning activities


was done with the help of the assigned Head Teacher. Post
Conference with concerned teachers and feed backing followed
after which. Lesson plans are checked regularly to enhance
effective learning, notations and suggestions are written to
remind them whatever are not very acceptable and needs to be
improved.

12.2. Regular checking of students’ academic achievements based


on the MPS of all subjects in every grading period.

12.3. Obligating teachers to attend In-Service trainings, LEAP


sessions, seminars and workshops to enrich their teaching
competencies and update them on the new strategies and
innovations

12.4. Encouraging the teachers to grow professionally to keep


abreast with the global innovativeness in technology and
computerization development program

12.5. Recommending teachers for promotion and reclassification as


such for Master Teachers, Head Teachers, Teacher-II, and
Teacher-III positions.
12.6. For the participation and involvement of students in
academic, socio-cultural, sports competitions. and other
activities, the following were employed:

12.6.1. Activation of school governance through the


Supreme Student Government, School Clubs and
Class Organizations.

12.6.2. Coordinating with other government agencies by


participating in culmination activities and
contests, such as DOH initiated nutrition in July,
Buwan ng Wika in August, Science and Math in
September, Intramurals in February, English
Festival in February, Christmas Festivities,
barangay and town fiesta celebrations.

12.6.3. Topping the School Governing Council in all


activities.

C. GOVERNACE

C.1. Staff Development

All teachers attended scheduled conferences called for by the Principal for purposes
of information dissemination, feed backing and consultation to improve teacher efficiency.
Conferences are held after four O’clock or on Saturday to ensure that all classes wee
delivered daily to adhere to the implementation of “ Time on Task”.

The teachers attended seminars, workshops, trainings and LEAP sessions as


required by higher offices to enhance their teaching capabilities and update their
knowledge of current thrusts and changes in the curriculum. The following are specifically
accomplished:

1. Sent 1 Science teacher to seminar on “Inclusive Education” on


May 1-3, 2013

2. Sent all the Grade 8 teachers to “K to 12 Building Skills Training in LNU


Tacloban on May 2013

3. Sent all Grade 7 Science teachers to Philippine Organization of Science


and Technology Educators (POSTE) Regional Seminar on August 23,
25, 2013

4. All teachers participated in 1st LEAP Session of Sogod Districts I and II


on Sept. 7, 2013
5. Sent 4 MAPEH teachers to the National Conference Workshop on
Culture and Arts Education for Teachers on Oct. 4-6, 2013.

6. Conducted a School-Based Two-Day Training on Assessing Students’


Learning held at the Computer Laboratory, Sogod National High School on
Oct. 30-31, 2013.

7. Sent 1 teacher participant who became champion in the oration contest


for teachers during the DepED Regional Anniversary

At the end of the semester, all teachers attended the Mid-year Program Review and
Evaluation (MPRE) with various concerns raised by teachers in the implementing
guidelines specially in the students tracking system and assessing and analyzing the given
test exams

The Principal, School Heads, and other focal faculty attended District Inter-face
conference, seminars/forums called by higher offices to update managerial capabilities and
keep abreast with current trends in the system.

C.2. Curriculum Development

1. All teachers were required of their Portfolio filed for reference


at the end of the school year.

2. All teachers were required to submit their daily lesson plans


with integration on the different subjects including Makabayan.

3. Class program was framed rising the new curriculum at its


specification.

4. All teachers were required to properly structure their rooms


and make them more conducive to learning.

5. All teachers were required to submit formative test notebooks


with their lesson plan to show the extent of mastery of the
lessons presented through the frequency of errors recorded
after the evaluation. The summative test notebook was also
required to show that questions were properly distributed
according to skills to be developed.

6. All students’ report card were released to parents after every


grading period during the homeroom PTA meeting.
7. Students tracking system was intensified to find out reason why
students drop-out from school collaboratively, parents and
teachers work hand in hand to help solve problems of students
who are SARDOs.

C.3. Management of Resources

1. Physical Resources

a. Conducted annual inventory of property of all equipment,


materials and furnitures including those that are donated.

b. Gathered all broken chairs and furniture for repair during the
Brigada Eskwela.

c. The property custodian segregated all usable and unusable


books, dilapidated materials and books which are declared
condemnable.

d. Kept the school safe and clean.

e. Provide enough books and chairs to students.

C.4. Highlights/Events Conducted/Participated in

1. Conducted Acquaintance Party and Induction Program on July 19,


2013 spearheaded by the SSG

2. Conducted English Festival on February 14, 2014 sponsored by


English COP (Community of Practice)

3. Conducted a Culmination Program in Buwan Ng Wika sponsored by


Filipino COP

4. All SSG Officers and Advisers participated in Edmund Rice


Leadership Seminar on September 7, 2013 at Shalom Park,
Malitbog, So. Leyte

5. All SSG Officers and Advisers participated in the 3-Day Leadership


Training sponsored by SACEDA Youth Lead on Oct. 4-6 at Silago, So.
Leyte

6. Celebrated World Teachers’ Day on October 2013 spearheaded by


the SSG.
7. Conducted 2-day Intramurals on January 17-18, 2014 spearheaded
by the SSG

8. Conducted Miss Science Explorer 2013 sponsored by Science COP

9. Conducted Coastal Clean-Up: A Closer Encounter to the


Environment on Dec. 19, 2013
