You are invited to take part in a research study to the Effects of the Accessibility of Smartphones
to the Grade 12 Students of St. Paul’s School of Ormoc Foundation Inc. We would like to interview
you to ask you about your ideas on [research topic]. This research is part of our Applied Research
subject at Saint Paul’s School of Ormoc Foundation, Inc.

Before you decide whether to take part in the study it is important that you understand what the
research is for and what you will be asked to do. Please take time to read the following information
and discuss it with others if you wish. It is up to you to decide whether or not to take part. If you
decide to take part you will be given this information sheet to keep. You will also be asked to sign
a consent form. You can change your mind at any time and withdraw from the study without giving
a reason.

The standard of care you receive will not change whether or not you decide to participate in this
study. You are welcome to phone us if you would like any further information.

You have been chosen because you are one of the students in the senior high school, specifically
in the grade 12, that have been given access to smartphones during class hours. The study will
involve 15 participants, who will all be interviewed separately. Initial interviews shall be done using
an open ended survey. Interview questions will be given ahead to serve as your guide. Your
answers in the survey sheets will be collected within a week after receiving the survey
forms. Your answers in the open ended survey will be kept with utmost confidentiality.

If there is a need for further information or clarification about your answers, a face to face interview
will conducted for a maximum of 20 minutes only. If you choose to take part of this study we may
organise a location for the interview convenient to you.

The information gained from this research will be used to make recommendations for best
practice, offer insights into the experiences and may also lead onto further studies on the effects
of accessibility of smartphones to the grade 12 students.

The face to face interview will be recorded on audio devices and then transcribed onto a computer.
The audio devices and survey sheets will be stored in a locked secure place at all times and the
computer data will be protected from intrusion also. The audio files will be deleted at the end of
the study. Your response will be treated with full confidentiality and anyone who takes part in the
research will be identified only by code numbers or pseudonyms. You can request a copy of the
interview transcript if you wish.

At the end of the research we will write a report and the results may be published in peer reviewed
journals and conference presentations. No research participant will be identifiable from any
publications.

5. The procedures regarding confidentiality have been clearly explained (e.g. use of
names, pseudonyms, anonymization of data, etc.) to me. 


data collection have been explained and provided to me.
7. The use of the data in research, publications, sharing and archiving has been
explained to me. 

8. I understand that other researchers will have access to this data only if they agree to
preserve the confidentiality of the data and if they agree to the terms I have specified 
in this form.
