RESEARCH INSTRUMENT
Dear Respondent,

I understand that I am being asked to participate in a research study entitled “THE LEVEL OF
SATISFACTION ON THE POLICE COMMUNITY RELATION AMONG THE
RESIDENTS COVERED BY THE PARIAN POLICE STATION”. This study will
determine the level of satisfaction on the offered by our police personnel in the community
towards its stakeholders of Parian Police Station in terms of police community relations.

If I agree to participate in the study, I will be made to answer a questionnaire for approximately
15-20 minutes about the satisfaction of police community relation. I will answer the
questionnaire during my most convenient time and place. I am aware that the questionnaire will
not bear my name or any information that divulge my personal circumstances. I understand that
the researchers will not give any compensation as an exchange of my participation. There are no
risks associated with the study except for the time I will consume in answering the research
questionnaire.

I was informed by the researchers that I can only participate on this study if I am a resident
currently enrolled in the University of the Visayas. I recognized that my participation in this
study is purely voluntary and I may withdraw from participating at any time I wish. If I decide to
discontinue my participation in this study, I will continue to be treated in the usual and
customary fashion.

I understand that all data will be kept with confidential. However, this information may be used
in public or presentations.

I understand that if I sustain injuries from my participation in this research undertaking, I will not
be automatically compensated by the College of Criminal Justice Education.
