Residency Directors (RDs) have begun to look for evidence of assimilation and exposure to the U.S. healthcare system when
evaluating IMGs. One of the most effective means to measure assimilation has been through the length of an IMG’s exposure
to U.S. healthcare and culture before the start of residency. In two landmark studies, RDs divulged that recent pre-residency
clinical experience & performance on standardized examinations (Part; Gayed), along with performance during the actual
residency interview (Part), are the most important predictors of IMGs performance during residency. Consequently, RDs look for
pre-residency clinical experience, supported by Letters of Recommendation from practicing U.S. Physicians; 73% percent of the
directors found Letters of Recommendation from a foreign country to be useless (Part). Furthermore residency programs ranked
U.S. clinical performance (grades and failures, if any) slightly higher than USMLE® performance (scores and attempts; AAMC, 2007).
Today’s IMGs have to compete with a greater number of well assimilated applicants who are applying to the same primary care
residency programs by showing that they can fully and immediately integrate into U.S. medical internship.

Begin With the End in Mind

Synopsis: Residency programs are requiring IMGs to document U.S. hands-on clinical experience in greater numbers every year. The majority of programs do not consider research,
abstracts, presentations, publications or observerships as hands-on clinical experience. Consider that an average of 2.5 Abstracts, Presentations and Publications were reported by
those who matched, and similarly, 2.7 by those who did not match, according to the National Resident Matching Program’s 2009 Charting Outcomes: therefore, there is not any
real statistical benefit to adding research to a medical residency application. AmeriClerkships Medical Society offers objectively evaluated, insured, hands-on clinical experiences
in five different formats: Teaching Hospital Guarantee Verified, Teaching Hospital Guarantee, Hospital Guarantee, Nursing Home Guarantee, Premium Elective Specialty, Premium
Elective Specialty with 100% Inpatient Hospitalist Experience, and Teaching Attending Physician.

Sample Medical 1. University of Buffalo, Family Medicine Residency requires that IMG applicants have six months of U.S. clinical experience
inpatient OR outpatient, in the past 2 years.
Residency 2. University of Minnesota, Mankato, Methodist, North Memorial, Smiley’s, St. John’s, and St. Joseph’s, Family Medicine and
Community Health requires a minimum of one year of U.S. clinical experience or rotations.
Prerequisites for 3. Floyd Medical Center, Family Medicine Residency requires IMG applicants to have completed at least one year of documented
clinical training in the United States as well as three Letters of Recommendation from faculty who have directly supervised
IMG Applicants clinical performance.
4. University of California, San Francisco, Family Medicine Residency states that, “Since clinical and cultural preparedness are
also a key to success here, prior clinical experience in the United States is helpful. Letters of Recommendation from these
experiences will further strengthen an IMG applicant’s documentation of overall readiness to begin residency training
in the U.S.”
5. Oregon Health and Science University, Internal Medicine Residency states that, IMGs must be ECFMG® certified at time of
application, must have at least one year of clinical experience in the U.S. or in a health care system similar to the U.S. and
have graduated from medical school within the last six years. Please note that externships, observerships, or research work
do not count as clinical experience.
6. Duke University, Family Medicine Residency requires applicants to have completed at least one year of clinical patient care in
an ACGME accredited program in the United States by day one of residency (an observership does not count).
7. Yale School of Medicine, Internal Medicine Residency seeks a minimum of eight weeks of hands-on direct clinical
experience in a U.S. or Canadian accredited teaching hospital. We will not interview candidates who have not obtained this
required clinical experience. Observerships without direct clinical responsibility do not meet these criteria.
8. University of Wisconsin Madison, Family Medicine Residency states that interview selection preference is given to applicants
who have clinical experience in the United States or Canada that is particularly relevant to Family Medicine (research or
observerships do not qualify as clinical experience).
9. Fletcher Allen Healthcare in Alliance with the University of Vermont requires that International Applicants MUST have worked
in clinical medicine in the United States within the last two years. NOTE: an observership does not fulfill this requirement.
10. University of Washington, Internal Medicine Residency requires eight weeks of clinical experience with direct patient
contact and accompanying Letter of Recommendation outlining the type of clinical experience.
11. Harvard University, Massachusetts General Hospital, Internal Medicine Residency considers only those with outstanding
Letters of Recommendation and a year or more of clinical or research experience in the U.S. or Canada for interviews.
12. University of Michigan, Internal Medicine Residency states an IMG must possess a letter from an Internal Medicine faculty
member who served as supervisor or instructor to the applicant, at a U.S. academic institution attesting to the applicant’s
clinical experience and competence at that institution.
13. University of Nebraska Medical Center, Internal Medicine Residency states that while research abilities and experience are
valuable, a strong emphasis is placed on recent clinical experience.
14. University of Rochester, Pediatrics Residency states that IMGs must complete at least eight weeks of clinical training in
an ACGME-approved pediatric program or a similar program in the United States, Canada or England. Observerships and
research are not acceptable as clinical training.
15. University of Pennsylvania, Department of Medicine looks for four weeks of U.S. hands-on clinical experience within the
past 6 months (observerships are excluded).
16. University of North Carolina, Chapel Hill, Family Medicine Residency gives preference to candidates with clinical experience
in the United States.

NEW FOR THE USMLE® AND THE MATCH

New Developments with the United States Medical Licensing Examination (USMLE®)

(October 2010) First, Step 1 will undergo content changes that will increase the clinical context of the exam and also prepare the examinee for the additions in science assessment
scheduled for Step 2 and Step 3. Additionally, increases in the number of biostatistics and epidemiology items have appeared in Step 2 in 2010 and will be added to Step 3 in 2011.

Second, there is the development of new item formats for Step 1, Step 2 CK, and Step 3. New literature items will test the examinee’s ability to interpret and comprehend
both scientific literature and pharmaceutical advertisements. Furthermore, new video and assessment activities are in development. These assessment activities will include
admissions orders and laboratory testing, for example, to show the progression of an individual patient case over time, further complementing the clinical accuracy of computer-
based assessments.

Third, the Step 2 Clinical Skills portion will be developed to expand the nature and level of challenges faced. Standardized patient interactions will be more authentic, which
will result in more demanding communication cases. Changes in patient notes will be added to assess the examinee’s clinical reasoning more effectively. This will include the
elimination of hand-written notes: All notes will be computer based beginning in the middle of 2011.

New ERAS® Feature of the Match: Supplemental Offer and Acceptance Program (SOAP)

(October 2010) The time period when unfilled Match-participating programs and unmatched applicants are allowed to directly contact each other (currently known as the
managed “Scramble”) will have new regulations and procedures taking effect for 2012 Match. Where in previous years, unmatched applicants were permitted to call, email and
fax representatives seeking acceptance into unfilled positions, now the certified applicant may only “reapply” through ERAS® and the NRMP’s® Supplemental Offer and Acceptance
Program (SOAP).

Through SOAP and ERAS®, unfilled Match-participating programs will be able to download application files and offer phone interviews with eligible unmatched applicants.
Unfilled Match-participating programs will submit preference lists through ERAS®. A program’s unfilled positions will be offered to candidates in order of preference until all
positions are filled or the preference list has been exhausted; programs will be able to add applicants to the end of their preference lists throughout Match Week. Therefore, the
improvements made to one’s application (hands-on clinical experience and/or advanced certifications) in the time leading up to SOAP will be vitally important, as well as an
applicant’s phone interview skills.

The AmeriClerkships SOAPTrack® 2012 Package will educate, strategize and prepare the applicant for SOAP through Virtual Residency Clinics as well as eight or twelve weeks of
supervised and insured clinical experience. Please see page 9 for details.

Teaching Hospital Guarantee Verified*– for medical students only

We guarantee that the trainee will be processed through the medical staff office of the hospital location. This ensures all requirements are met for future state licensure. (Twenty-
one states in the United States have strict regulations on where and how an international medical student completes their required clinical clerkships before graduation.)
AmeriClerkships will guarantee your ability as a qualified clinical trainee to enter at least one (1) teaching hospital that is recognized by the ACGME or AOA, and to round on at least
one (1) patient per week at that hospital (averaged over 4 weeks), along with an AmeriClerkships affiliated supervising teaching attending physician. The 4-4-4 Rule applies: four

A hospitalist is an emerging medical specialty which focuses on quality medical care to individuals while they are in the hospital. Because hospitalists typically don’t maintain a
private outside practice, their time is devoted primarily to caring for hospitalized patients. Hospitalists manage and coordinate the entire inpatient episode of care, working closely
with the patient’s primary care physician and/or specialists involved.

Minimum Qualifications for Enrollment with AmeriClerkships Medical Society:

• Medical school must be recognized by the Medical Board of California (www.medbd.ca.gov/applicant/schools_recognized.html),
or by the New York State Education Department of Education (contact NY State Board of Education)
• Must have passed USMLE® Step 1 or Step 2 CK to participate in Teaching Hospital Guarantee clinical experiences (students and graduates)
• Less than fifteen years since graduation from medical school (twenty years if extreme extenuating circumstances)
• Maximum seven total attempts on all USMLEs®

Clinical Prerequisites with AmeriClerkships Medical Society:

2. Signed HIPAA Business Associate Confidentiality Agreement
3. Clinical Authorization Letter and Medical School Dean’s Letter of Good Standing (students only)

5. Notarized proof of identity and legal status to be in the United States
6. Immunizations according to the Center for Disease Control
7. Criminal background check, conducted by AmeriClerkships Medical Society
8. Certificates in AmeriClerkships Medical Society IMG Orientation and Acculturation, Basic HIPAA Training, American Heart Association Basic
Life Support, Blood Borne Pathogens, and Basic First Aid
9. Financial good standing with AmeriClerkships Accounting Department
10. Up to date, professional and presentable Curriculum Vitae
11. Proof of personal health insurance
