Validated physical server is a candidate for virtualization?
Reboot server. Server stable after reboot?
Event log contains any critical hardware errors (e.g. disk, memory)?
Local administrator password tested?
Network access to P2V Mgmt server tested?
Network access to destination ESX server tested?
Ensure VLAN ID portgroup exists and is working in virtual infrastructure
Create/resize paging file on OS logical drive
Check to make sure CDROM on physical server is working
Ensure there is 300 MB free space on physical server OS partition (for hot migration)
If physical server has less than 264 MB memory, plan to use imaging tool
Open ports 445, 139 between physical server and Converter VM
Open ports 443 between physical server and VirtualCenter/ESX
Adequate space on target datastore?
Adequate resources on target ESX host / VI Cluster?
Downtime per SLA
Change request submitted and approved?
Customer notified of outage?
Customer contact name / phone / email
Customer data center escort contact name / phone / email
Customer network/security admin contact name / phone / email

Remove hidden devices and old NIC, SCSI, adapters/drivers etc. for devices that you are
sure are tied to the old physical server and not needed anymore. These should be light
in color i.e. hidden devices.
