Notice to Doctors regarding the GROASSIST v2 system.

Dear Doctors,

Please be informed that the GROASSIST v2 will be launched on July 1st 2009. As part of the
GROASSIST v2 system, the on-line submission of foreign worker medical examinations
results had been revamped for improvements.

 Login ID and Password
 Panel doctors are required to login using the current user ID and Password when they first
time login to the GROASSIST v2 and then follow the instruction given in the website.

Date of Physical Examination

 Panel doctors are required to record the "date of physical examination" of the foreign
worker within 24 hours of examining the foreign worker.

On-Line Medical Form

General
 Some of the checkboxes within the on-line medical forms had been pre-filled for ease of
data entry. Doctors are required to change the pre-filled checkboxes appropriately according
to the findings of the medical examination.
 The new medical form has been separated into a few pages in order to ease the submission
of the information.

Part I – Declaration by Doctor

 Panel doctors are required to agree with a few declaration indicate in GROASSIST V2 before
doctors are allowed to certify the worker.
 This “declaration” indication is reflected as a checkbox in the first page of the foreign worker
screen as shown below. If the “declaration” checkbox is not checked, you will NOT be able
to transmit your certification.

 Information
 The foreign worker’s information and medical information can be accessed when the specific
button is clicked as shown below:

 Part II – Medical History
 Doctors are required to record whether the foreign worker had taken any medication/drugs
within the last 2 weeks before physical examination.
 This field is reflected as item 3 in Part II – Medical History – Category 2 of the on-line
medical form. This is a YES/NO field with no dates required.

 Part III – Physical Examination & Investigations
 Doctors are now required to record the last menstrual period date (item #5, in the format of
DD-MM-YYYY) for female foreign workers in Part III - Physical Examination of the on-line form.
Please record the first day of the month (e.g. 01-04-2009 for April, 2009) if the date is not
known. Alternatively, please record the first day of the year (e.g. 01-01-2009 for year 2009) if
the date and month is not known.
 If you leave the last menstrual period date empty, you MUST fill in the Part III comments (item
#15) to explain why the last menstrual period date is not available in order to transmit your
result.

This statement is
referring to female

 Part III – Lab & X-Ray Results
 Doctors are required to visit the laboratory results and x-ray findings pages within the new
GROASSIST V2 before they are allowed to transmit the certification of the foreign worker.
 If doctors attempt to transmit the worker certification without visiting the laboratory results and
x-ray findings pages, they will be prompted to do so.

 Part IV – Certification By Doctor
 If doctors certify foreign worker as "SUITABLE for Employment in Malaysia" in item #14 in
Part IV of the form, the "Reason if unsuitable" (item #16) must be left blank. If not, doctors
are not allowed to proceed with the certification process.
