Two Civil Cover Sheets were included in the docket, none of them includes a valid signature of the
Plaintiff, as required for opening a Civil Docket.

a. The first Civil Cover Sheet JS-44 (Rev 1/11 DC) states:
This form, approved by the Judicial Conference of the United States in September 1974, is required for
the use of the Clerk of Court for the purpose of initiating the Civil docket sheet. Consequently a Civil
cover sheet is submitted to the Clerk of Court for each civil complaint filed.

On page 2, instead of signature of Attorney of Record (Orly Taitz) initials were inscribed of
“NCD” with the date of “2-16-11” – the date that the complaint was received by mail in the US
District Court, Washington DC.

b. The second Civil Cover Sheet JS-44 (Rev 12/07) bears no signature of Attorney of Record at all.
