MINUTES OF SPECIAL MEETING OF THE ICE PLANT EMPLOYEES HELD ON


MARCH 19, 2011.

A. CALL TO ORDER:

Engr. Joey Baesa, Pollution Control Officer , called the meeting to order at 1:428 p.m. Ma.
Emmylou F. Magno, Human Resource Officer act as Secretary and recorded the minutes. A
quorum of Employees was present, and the meeting, have been duly convened, was ready to
proceed with business.

B. PRESENT:

The following workers were present during the meeting;

C. TOPICS OF THE DAY:

Engr. Joey Baesa reminded to the workers to improve their performance in terms of
utilizing the time properly. He explained to the group the causes and effects of the improper
handling of the specific task. Engr. Baesa stressed out that the compressor should maintain its
lower pressure reading in terms of load as this may improve the system. With the proper
handling of the compressor, the electric consumption maybe reduced.

It has been discussed that the machine operators should only stay at the machine room, so
that he may observe the machine and ensure that no breakdown may arise. The operators
should avoid leaving the machine room and the preparation of the DR shall be tasked to
another worker. It was also emphasized that a maximum of 8 hours work shall be observed,
and the Ice blocks should be harvested with a ratio of 20 ice blocks per hour or total of 160
ice blocks in 8 hours. The Machine Operator being the leader of team should see to it that
proper harvesting should be observed.

Mr. Cris John dela Cruz commented that workers are sometimes caught sleeping and they
are being called to get back to work which sometimes causes delay in the production.

The operators inform the body that only one (1) gas musk is issued and Engr. Baesa
suggested that additional of 2 gas musk should be provided. Aside from the gas musk Mr.
Alberto Requinto inform the workers that 2 sets of Personal Protective equipments will be
issued every year, but 1 set of the PPE (overall, rubber boots, gloves) shall be charged,
deductible from their salary. But Mr. Requinto make it clear that every worker should
comply the company regulations that they should always observe wearing their uniforms.
Incase worker caught not wearing proper uniform a corresponding penalty shall be imposed
for 2 offenses but subject for suspension or termination incase successive offense committed.

Mr. Requinto also inform the body that he will look forward to provide locker for the Ice
plant workers.

D. .ADJOURNMENT:

There being no matter to be discussed, the body, on motion by Mr. Alberto Requinto,
adjourned the meeting at 4:07 p.m

Contract Agreeement

This contract has been made on this 14th day of March 2011 by and between EDGARDO
DADO with postal address at Tumaga, Zamboanga City and MELINA C.
REQUINTO President/CEO AMR Trade and Industrial Development Corporation, Cawit,
Zamboanga City with the following terms and conditions:

That a contract of work for the Interior and Exterior Painting of Requinto’s mansion is
amounted to php 105,000.00 to be completed within ___________days from the date hereof.

That upon the commencement of the painting job the Painting Contractor is allowed to avail of a
cash advance of Php ________________, but not more than 70% of the contract amount. The
parties further agreed that the cash advances shall also be limited base on the completion of the
painting works done.

That the remaining 30% shall be paid upon the completion of the Painting works, hence in the
event of some defects incurred on the job stated above, the Contractor shall be held liable and is
required to perform the necessary finishing touches on the said area with regards to the painting
jobs within the period of 6 months.

Subject: Notice for Cash Advance Limits for Security Guards

Notice is hereby given to all Security Guards of Anchorline Security Agency that effective
March I am very pleased to accept the position of HUMAN RESOURCE OFFICER of your
company, AMR Trade and Industrial Development Corporation. Thank you for the opportunity. I
am eager to make a positive contribution to the company and to work with everyone on the
ATIDCO team.

As stated in the Administrative Order No. 2011-001 , my salary will be Php10,000 and SSS,
Pag-ibig and Philhealth, Monthly representation Allowance of Php1,500 and Gas allocation of
Php 1,500 allowance will be provided.

Thank you again for giving me this exciting opportunity. I look forward to assume the HRO on
March 1, 2011 becoming a member of such a dynamic team.

As part of my duties and responsibilities as the HRO, I would like to inform you that I am setting
up a DATABASE of all Contractual, Casual and Regular Employees of ATIDCO, be it in the
Fishing, Engineering, Iceplant Workers, Security agency, Resort and other Departments.
