While there are many different varieties of heart diseases,many a times


they are easily overlooked,which can cause delay from not seeking
treatment for this conditions at a early stage.Some of the common
symptoms may also be from allergies or heartburn, which is not related

Arrhythmia
Arrhythmias (or dysrhythmias) are problems that affect the electrical
system of the heart muscle, producing abnormal heart rhythms. They can
cause the heart to pump less effectively.
The heart has four chambers. The top two are the atria, and the lower two
are the ventricles. Normally the heartbeat starts in the right atrium when
a special group of cells sends an electrical signal. (These cells are called
the sinoatrial or SA node, the sinus node or the heart’s “pacemaker.”)
This signal spreads throughout the atria and to the atrioventricular (A-V)
node. The A-V node connects to a group of fibers in the ventricles that
conduct the electrical signal. The impulse travels down these specialized
fibers (the His-Purkinje system) to all parts of the ventricles. The
electrical signal must follow this exact route for the heart to pump
properly.

Under some conditions almost all heart tissue can start a heartbeat. In
other words, another part of the heart can become the pacemaker.

An arrhythmia occurs…
* when the heart’s natural pacemaker develops an abnormal rate or
rhythm.
* when the normal conduction pathway is interrupted.
* when another part of the heart takes over as pacemaker.

What is a normal heart rate or pulse?

The heart contracts (beats) as the electrical impulse moves through it.
This normally occurs 60 to 80 times a minute when a person is at rest.
The atria contract a split-second before the ventricles. This lets the atria
to empty their blood into the ventricles before the ventricles contract.

What causes arrhythmias?

Many arrhythmias have no known cause. However, a number of factors


can contribute to arrhythmias. These include coronary artery disease,
high blood pressure, diabetes, smoking, excessive use of alcohol or
caffeine, drug abuse and stress.

Certain substances, including some over-the-counter and prescription


medications, dietary supplements and herbal remedies are known to
cause arrhythmias in some people.

Atrial Fibrillation
What is atrial fibrillation (AF)?

Abnormal heart rhythm or cardiac arrhythmia is most commonly known


as atrial fibrillation:Atrial fibrillation is a disorder found in about 2.2
million Americans. About 15 percent of strokes occur in people with atrial
fibrillation.
The likelihood of developing atrial fibrillation increases with age. Three to
five percent of people over 65 have atrial fibrillation.
Atrial fibrillation involves the two larger, upper chambers of the heart and
fibrillation is the medical term used to describe "quivering" of the affected
muscles of the heart.

Before we take a more detailed look at atrial fibrillation, we must first


understand a little about the structure of the heart to better understand
what is happening with this condition and how we treat it.
The heart is composed of four chambers; as we have already said, the
two larger chambers, known as atria, sit on top of two bigger chambers
known as ventricles and each atrium is connected to the lower ventricle.
The atrium's job is to take in blood after it has passed through the lungs
and which is then rich in oxygen waiting to be delivered to the rest of the
body. The atrium then contracts to force this blood into the lower
ventricle. The ventricle is smaller than the atrium but much more
powerful and when it contracts, this forces the blood out of the heart and
around the body under pressure.

It is the double contraction of the atria followed by the ventricles which


forms the typical heart beat we hear as a double thud – "duh-dum".

With a-fib, the regular double-beat becomes uncoordinated and this


leads to blood not being pumped to the ventricles effectively.

The actual cause is when the the electrical signal which coordinates the
heart is disrupted or not passing across the heart as it needs to in order
to trigger the contraction of the muscles. Disruption is frequently caused
by the electrical signal being drowned out by electrical impulses
generated elsewhere by the atria. This creates the arrhythmia or irregular
heartbeat and an episode may last for a few moments or for an extended
period of time, sometimes for years until it is detected by healthcare
professionals.

A-fib is not usually life threatening but a range of symptoms may present
themselves and are not generally pleasant - fainting, palpitations, pain in
the chest or more seriously, congestive heart failure. Having atrial
fibrillation will lead to a higher risk for other illnesses such as strokes
due to blood clots forming from the blood which pools in the atrium
because of infective pumping into the ventricle.

How is atrial fibrillation treated?

Several approaches are used to treat and prevent abnormal beating:

Medications are used to slow down rapid heart rate associated with AF.
These treatments may include drugs such as digoxin, beta blockers
(atenolol, metoprolol, propranolol), amiodarone, disopyramide, calcium
antagonists (verapamil, diltiazam), sotalol, flecainide, procainamide,
quinidine, propafenone, etc.

Electrical cardioversion may be used to restore normal heart rhythm with


an electric shock, when medication doesn’t improve symptoms.

Drugs (such as ibutilide) can sometimes restore the heart’s normal


rhythm. These drugs are given under medical supervision, and are
delivered intravenously, usually in the patient’s arm.

Radiofrequency ablation may be effective in some patients when


medications don’t work. In this procedure, thin and flexible tubes are
introduced through a blood vessel and directed to the heart muscle. Then
a burst of radiofrequency energy is delivered to destroy tissue that
triggers abnormal electrical signals or to block abnormal electrical
pathways.

Surgery can be used to disrupt electrical pathways that generate AF.

Atrial pacemakers can be implanted under the skin to regulate the heart
rhythm.
American Heart Association Recommendation for Stroke Prevention

Treating atrial fibrillation is an important way to help prevent stroke.


That’s why the American Heart Association recommends aggressive
treatment of this heart arrhythmia.
Drugs are also used to help reduce stroke risk in people with AF.
Anticoagulant and antiplatelet medications thin the blood and make it
less prone to clotting.

Warfarin is the anticoagulant now used for this purpose, and aspirin is
the anti platelet drug most often used.Long-term use of warfarin in
patients with AF and other stroke risk factors can reduce stroke by 68
percent.

Physicians differ on the choice of drugs to prevent embolic stroke ―


stroke caused by a blood clot. It’s clear that warfarin is more effective
against this type of stroke than aspirin. However, warfarin has more side
effects than aspirin. Examples include potential bleeding problems or
ulcer.
Patients at high risk for stroke should probably be treated with warfarin
rather than aspirin unless there are clear reasons not to do so.

Aspirin is the standard treatment for patients at low risk for stroke and
under 75 years of age.
Some of these arrhythmias are medical emergencies as they can result in
sudden death.They may also predispose the patient to life-threatening
stroke or embolism.Early diagnoses of these medical conditions is the key
to life long healthy life.

Stroke

A stroke can be a very scary medical emergency. It can be life-


threatening if it is not treated immediately. A stroke is the loss of blood
flow to any area in the brain. The lack of blood supply to the brain is
usually caused by a blocked or burst blood vessel. As a result of the lack
of blood flow to the brain, the affected area will lose function. This can
lead to the inability to move one or more limbs on one side of the body,
the inability to understand or formulate speech, or the inability to see one
side of the visual field. A stroke can cause permanent damage; therefore
it is important that stroke treatment is sought for immediately.

Strokes are classified into two different groups: ischemic and


hemorrhagic. In an ischemic stroke there is interruption of blood flow to
the brain.
An ischemic stroke happens when there is an obstruction of a clot formed
locally, because of an embolism formed somewhere in the body due to
pooling of blood.

About 80% of strokes are due to an ischemic event, while the others are
classified as hemorrhagic strokes.

Hemorrhagic strokes happen when there is a rupture of a blood vessel or


an abnormal vascular structure. Some hemorrhagic strokes will even
develop inside an area of ischemia. It is unknown how many hemorrhagic
strokes actually begin as ischemic strokes.

Because stroke injures the brain, you may not realize that you are having
a stroke. To a bystander, someone having a stroke may just look unaware
or confused. Stroke victims have the best chance if someone around them
recognizes the symptoms and acts quickly.

The symptoms of stroke are distinct because they happen quickly:

•Sudden numbness or weakness of the face, arm, or leg (especially on


one side of the body)

•Sudden confusion, trouble speaking or understanding speech

•Sudden trouble seeing in one or both eyes

•Sudden onset of difficulty in walking, dizziness, loss of balance or


coordination

•Sudden severe headache with no known cause

Worldwide, stroke is the second leading cause of death, responsible for


4.4 million (9 percent) of the total 50.5 million deaths each year.

Stroke is the No. 3 cause of death in the U.S., behind heart disease
(with which it is closely linked) and cancer.

Stroke affects more than 700,000 individuals annually in the United


States (approximately one person every 45 seconds).

About 500,000 of these are first attacks, and 200,000 are recurrent
attacks.

Someone in the U.S. dies every 3.3 minutes from stroke

Stroke is the leading cause of disability among adults in the U.S.

More than 4 million people in the United States have survived a stroke
or brain attack and are living with the after-effects.

Four out of five families will be somehow affected by stroke over the
course of a lifetime.

Current statistics for stroke survival rates are:

•10 percent of stroke victims recover almost completely.

•25 percent of stroke victims recover with minor impairments.

•40 percent of stroke victims experience moderate to severe impairments


requiring special care.

•10 percent of stroke victims require care in a nursing home or other


long-term care facility.

•15 percent die shortly after the stroke.

•7.6 percent of ischemic strokes and 37.5 percent of hemorrhagic


strokes result in death within 30 days.

•While subarachnoid hemorrhage (SAH) represents only about 7 percent


of all strokes, it is the most deadly ― with more than a 50 percent fatality
rate. Of the survivors, approximately half will suffer permanent disability.

•22 percent of men and 25 percent of women die within a year of their
first stroke.

Economic Cost of Stroke

The total cost of stroke to the United States is estimated at $43 billion
per year.

The direct costs of medical care and therapy are estimated at $28
billion per year.

Indirect costs from lost productivity and other factors are estimated at
$15 million per year.

The average cost of care for a patient up to 90 days after stroke is


$15,000.

For 10 percent of patients, the cost of care for the first 90 days after a
stroke is $35,000.

These facts about heart disease and stroke are to scare you or shock you,
But they are real and you must be aware of them if you want to keep
yourself and your family safe from these two deadly killers.

While both of these diseases are out there, we can follow simple steps to
prevent them from occurring. There are solutions to these problems, so
with a few changes in our daily habits we can protect ourselves.

Section 2 – The General Solution


Better Health Management By Understanding Risk

There are several risk factors for heart disease (heart attack and stroke)
that you can control or that can be treated: high cholesterol, high blood
pressure, smoking, inadequate exercise, obesity, and diabetes. These risk
factors are all interrelated.
High cholesterol has no symptoms unless you have the results of a blood
test to measure both LDL ("bad" cholesterol) and HDL ("good"
cholesterol).

As cholesterol builds up in your blood vessels, the risk of heart attack


and stroke increases. Whole arteries can be blocked, or bits of the
buildup can break off and travel to other parts of the body preventing
blood flow to vital organs.

Find out what your cholesterol levels are and work with your healthcare
professional to lower them if necessary. A diet low in saturated fats, trans
fats, and cholesterol is usually recommended. Exercise also helps, but
some may need medication to lower the level of cholesterol in the blood.

High blood pressure also has no symptoms which has earned it the name
"Silent Killer". It makes your heart work harder which makes both your
heart and your arteries susceptible to injury and/or disease.

If you are 20 or more pounds overweight, you are more likely to have
high blood pressure than someone with a healthy weight for their age
and height. In your periodic visits to your healthcare professional, it is a
good practice for them to take your blood pressure, regardless of the
reason you are visiting.

You can also get your blood pressure taken at most pharmacies and
senior citizen centers. Ask for the numbers and see if they fall within the
norms set by the American Heart Association for healthy blood pressure
(120/80 mm Hg). If your blood pressure is high, work with your

healthcare professional to make a plan to lower it as soon as possible.


The plan may include lifestyle changes like weight loss and regular
exercise, reducing fat and salt in your diet, or taking medication in
addition to diet and lifestyle changes.

Smoking is another factor that can be controlled. Think about not only
the cigarettes or cigars that you smoke, but also the second hand smoke
from others that also increases your risk. The good news is that research
shows us that when you stop smoking, no matter how long you have
smoked your risk for heart attack and stroke begins to drop. Risk is cut in
half after one year without smoking, and continues to drop until it is as
low as a non-smoker.

Inadequate exercise is a risk factor that can be eliminated with as little as


10 minutes a day of physical activity for most healthy people. Walking,
dancing, gardening, and housework are activities that help-you don't
have to become a professional athlete to reduce your risk of heart
disease with exercise.

If 30 minutes seems too much to do at one time, start with 10 minutes of


exercise and then work up to more. Check with your healthcare
professional before starting an exercise plan.

Obesity or being overweight can be dealt with by a sensible plan of eating


and exercise that will help you reach a healthy weight and stay there.
Avoid fad diets that are advertised as helping you lose weight fast. To
stay off, the weight must come off slowly - no more than one to two
pounds per week.

Your healthcare professional or a registered dietitian can help you set up


a sensible plan of not less than 1200 calories a day. It will take time, and
having the support of your family and/or friends helps you stay with it for
the long haul.

Diabetes is also becoming an increasing problem in this county and is no


longer a disease confined to middle age and overweight people. When
women with diabetes were compared to women without diabetes, the risk
of heart disease increases two to four times. Diabetes is treatable.

If you have a family history of diabetes, ask your healthcare professional


for a fasting blood sugar test. If you develop diabetes, you must have
regular medical checkups and work closely with your healthcare
professional to manage your diabetes and eliminate other risk factors
that can be controlled.

There are risk factors that we cannot control and that we need to be
aware of. Specifically, we cannot control our age, gender, family history
and race. As we get older, the risk to develop heart attack or stroke. Men
have a greater risk of heart attack than women, and they have them at a
younger age. Women who are pregnant, take birth control pills and
smoke, or have high blood pressure have a higher risk of stroke.

If any of your family members had history of heart attacks or stroke, you
may be at risk, too. Heart disease risks are higher among Mexican
Americans, American Indians, and native Hawaiians, due in part to higher
rates of obesity and diabetes. African Americans have a higher risk than
Caucasians.

Regardless of race, most people with a strong family history of heart


disease or stroke have at least one other risk factor. Control your risk
factors if you can, and manage your stress. Increased stress can lead you
to overeating or having that extra cigarette and other activities that
increase your risk for heart attack and stroke. It is also true that a
previous heart attack or stroke can make us susceptible to another
stroke.

Early Diagnosis

In the case of heart disease and stroke getting an early diagnosis is vital
to dealing with the disease and will greatly increase the chances for you
to experience a speedy recovery.

The first thing that you can do to ensure help secure an early diagnosis of
heart disease is to make frequent visits to your healthcare professional.

oftentimes only available at large doctor's offices and hospitals, but they

Although humans have one heart, from a functional point of view we can think
of two pumps in series connected by the pulmonary and systemic circulations,
the left heart and the right heart. Each of these “hearts” is further divided into
two chambers. So the heart has four chambers in total, left and right atrium, as
well as left and right ventricle. Atria, the pre-chambers, mainly function as the
helpers of the ventricles by filling them with more blood before their
contraction. The function of the right side of the heart is to collect de-
oxygenated blood, in the right atrium, from the body and pump it, via the
right ventricle, into the lungs (pulmonary circulation) so that carbon dioxide
can be dropped off and oxygen picked up (gas exchange). The left side
collects oxygenated blood from the lungs into the left atrium. From the left
atrium the blood moves to the left ventricle which pumps it out to the body.
On both sides, the lower ventricles are thicker and stronger than the upper
atria, since they produce the greatest part of the force needed to pump the
blood. The muscle wall surrounding the left ventricle is thicker than the wall
surrounding the right ventricle due to the higher force needed to pump the
blood through the systemic circulation.
Normal conduction pathway in the heart starts in the SA node, located in the wall of the right atrium, because this is where the normal
rhythmical impulse is generated and is then conducted to the atrioventricular (AV) node by the internodal pathways. From here the AV
bundle conducts the impulse from the atria into the ventricles where the left and right bundle branches of so called Purkinje fibers
conduct the cardiac impulse to all parts of the ventricles. Impulses are also conducted by the atrial and ventricular muscle fibers, the
only difference is in the speed of conduction which is much slower in this case than in the case of specialized conductive muscle
fibers. Another important thing to mention when we talk about impulse conduction is its slowing in the AV node. When the impulse is
generated in the SA node, it spreads through both right and left atrium via the atrial muscle and some conductive muscle fibers, and
atria simultaneously contract. But they do this just a fraction of a second before ventricles, because the impulse hasn’t yet spread
through them since the AV node delayed it for about 0.13 seconds. This delay is important because it allows time for the atria to empty
their blood into the ventricles before ventricular contraction begins, thus allowing for more blood to be pumped out.

P wave: During normal atrial depolarization, the main electrical vector is directed from the SA node towards the AV node, and spreads
from the right atrium to the left atrium. This turns into the P wave on the ECG. Duration :80ms

PR segment: The PR segment connects the P wave and the QRS complex.Duration:150 to 200ms

QRS complex :The QRS complex is a recording of a single heartbeat on the ECG that corresponds to the depolarization of the right
and left ventricles.70 to 110ms

ST segment:The ST segment connects the QRS complex and the T wave.80 to 120ms

T wave: The T wave represents the repolarization (or recovery) of the ventricles. The interval from the beginning of the QRS complex
to the apex of the T wave is referred to as the absolute refractory period. The last half of the T wave is referred to as the relative
refractory period (or vulnerable period).160ms

PR interval :The PR interval is measured from the beginning of the P wave to the beginning of the QRS complex.120 to 200ms

ST interval :The ST interval is measured from the J point to the end of the T wave.320ms

QT interval :The QT interval is measured from the beginning of the QRS complex to the end of the T wave. 300 to 440ms

U wave: The U wave is not always seen. It is typically small, and, by definition, follows the T wave.

the large, bulky equipment that they once all were. Even some of the full
12-lead recorders today are small, portable, and operate through
desktop and laptop computers.
Now there are even smaller, self-contained, simple, one lead recorders
that are handheld, pocket-sized, battery operated, and have their own
built-in displays.

These new, low-priced, 1-lead handheld ECG monitors have become


available for personal, home, and sports use, much as with home blood-
pressure/pulse recorders or glucose testers for diabetics. They also are
suitable in some emergency situations but only when used by medical or
emergency personnel or for obtaining useful information while waiting
for emergency help to arrive.
