I hereby authorize THE Wyatt Violin Shop to initiate debit entries, and to initiate, if necessary, credit entries and any adjustments
for any debt entries made in error to the account selected above. The authority is to remain in full force and effect until THE
WYATT VIOLIN SHOP has received written notice of its termination or until rental contract is terminated.

RENTAL AGREEMENT:
The Wyatt Violin Shop Rental Agreement is subject to the terms on the reverse side of this form. The renter therein
agrees to abide by the terms and conditions of the rental agreement as described. Sizes may be exchanged as needed
with no loss of equity. Rental charge may vary with size of the instrument. The rental may be returned at any time
within the prescribed time limit. Upon return of the rental outfit, 100% of your rental equity (less taxes and fees) may
be applied to purchase the instrument outfit currently rented or 100% of your equity may be applied up to 50% of the
full purchase price of any non-rental instrument*. Rental monies accrued cannot be applied to accessories, repairs or
other charges. Note: outfits consist of an instrument, bow and case. The Wyatt Violin Shop guarantees the condition of
the instrument at the time of rental. Rental rates are subject to change and we will notify you if there is an alteration to
your monthly payment amount. Maintenance, insurance and a processing fee is included in the total monthly payment
at a rate of $7 for violin & viola and $15 for cello.
* Limited to the same type instrument as described on reverse side, Part 1 of this contract. (ex. violin-violin, viola-viola, etc…)

MAINTENANCE AND INSURANCE:

The Wyatt Violin Shop will maintain the instrument as follows:
1. The Wyatt Violin Shop will repair damage to the instrument or bow at no further charge provided the damaged
instrument or accessories are brought to The Wyatt Violin Shop. If an instrument is damaged beyond repair we will
replace it at no charge. If damage occurs a second time the renter is responsible for 60% of the repair costs. You will
be charged if any repair work is done outside this shop; both by the shop that performed the repair as well as by The
Wyatt Violin Shop if the work does not meet our standards.
2. The Wyatt Violin Shop reserves the right to assess repair charges for intentional damage to the instruments and
reserves the right to charge Customer for repair or replacement of Outfit components returned with excessive wear
and tear, up to but not exceeding the retail value of the Outfit components.
3. The Wyatt Violin Shop will supply replacements for broken strings at a discount of 50% off the list price.
4. The rental account must be paid and up to date to use the maintenance plan.
5. The maintenance plan does not cover lost or stolen items.

LATE FEES:

IMPORTANT – PLEASE READ: Rent is due on the due date. If payment is not received on the due
date a $5 late will be assessed. For each week the account remains unpaid an additional $5 late fee will be assessed.
The Wyatt Violin Shop reserves the right to require the return of the rented instrument package at any time the
account is past due. This includes picking up the instrument from the school or home where the instrument is kept.

THE WYATT VIOLIN SHOP RESERVES THE RIGHT TO USE ANY CREDIT CARD OR BANK ACCOUNT
LISTED ON THE REVERSE WITHOUT NOTIFICATION TO COLLECT SAID CHARGES. After 45 days of
delinquency the account will be turned over for collection for the full purchase price of the outfit plus any and all costs
of collection and late fees due. Any additional charges over and above the monthly rent do not add to the renter’s
equity. There will be a $35.00 charge (or actual bank fees if higher) for any check returned for insufficient funds.

BANKRUPTCY:
In the event of bankruptcy by the renter, the rental property remains the sole property of The Wyatt Violin Shop and
must be returned immediately. Any rental equity may still be used towards the purchase of an outfit as described above
with any balance due in full at that time.

RENTER SHIPPER INFORMATION (Special Rates)

All Violins and Violas ship for $18 nationwide. All 1/10-1/8 Cellos ship for $28 nationwide. All 1/4-4/4 Cellos ship
for $57 nationwide. All orders within the continental US are shipped UPS unless otherwise requested.
