To Whom it Concerns,
I am writing this letter because you will notice a gap in my employment. Thi
s gap is from an injury I received in 2005 in an accident. Since then I have und
ergone a couple of surgeries that resulted in a long recovery period.
I am currently receiving Social Security Disability until I heal completely
from my last surgery in November of 2008. It has been 2 years since that surgery
 and I have been released to go back to work with limitations.
I am a new resident of Arizona and I feel I must inform you of my physical l
imitations because of my disability. My limitations are only based on my ability
to lift anything over 10 lbs so as long as there is no heavy lifting I should b
e fine.
With all of this information I have given you I hope this does not hinder me
from working for your company. I felt I must divulge this information so there
is no misunderstanding if for some reason I cannot perform a heavy physical task
for you.
