Summary: Bloomington Hospital is considered as a secondary-care hospital that offers services like cardiac catheterizations and neurosurgery and is considered as a regional referral center for a multicounty area (Owen, Clay, Greene, Martin, Lawrence, Orange and Crawford). On the other hand, hospitals in Indianapolis provide services/care which secondary hospital like Bloomington cant provide. Bloomington doesnt really compete with other secondary-care hospitals like the Columbus Regional Hospital; they just enhance their status as a secondary-care hospital that offers specialized medicine. The Board of Directors of the hospital considers beginning an open heart surgery (CV surgery) and expands their range of services which will give the hospital a better reputation and status. But along with this suggestion comes the objections of most physicians; they think that the thought of this expansion will cost them to incur more debt, that the CV surgery will take a long time to hit break even and that most of the resources (space, staff) will be used up by the CV surgery. Now, the management is forced to decide if they will push through with the expansion or not, or research further on the pros and cons of the expansion. Point of View: Our group is taking the point of view of a concerned citizen of Bloomington. Main Problem: Whether to push through with offering CV surgery at Bloomingtons Hospital, to drop the idea completely or to research further about the pros and cons of having it in the hospital. Areas of Consideration: Pros: If the hospital carries on with offering CV surgery, lives will be saved; this is because the patients who need tertiary care will have to drive 50 miles from Bloomington to an Indianapolis Hospital. The status of Bloomington Hospital will be raised as compared to other secondarycare hospitals; Bloomington wouldnt be competing with other hospitals, they only want to keep up with the latest types of specialized medicines that should be available

to the public.

They would earn additional income and, at the same time, incur fewer expenses than they normally would if they were to offer CV surgery and reach the quota for the program called center for Excellence. (This is due to Medicare payments).

Cons: If they push through with the plan of offering CV surgery in Bloomington Hospital, other services might not be given as much importance to like before. Moreover, the hospitals resources would more likely be used by the CV surgery; hence, other services might not be performed properly. Most of the physicians and residents object with offering the CV surgery in Bloomington Hospital. This having said, we can say that they do not support the plan and implementing this plan would give the hospital administration a hard time. The hospital didnt involve other people who are concerned with the issue in the process. They only produced estimates, financial analyses and researches that would support CV surgery should be offered in Bloomington Hospital. They should have involved other people like the physicians, the public and the other counties who turn to Blooming for secondary care. Alternative Courses of Action: 1. They could push through with offering the CV surgery in the hospital, but they will have to deal with the issues that the public & the physicians will throw at them. 2. They can completely omit the idea of having CV surgery in Bloomington Hospital and just focus on improving other fields in medicine. 3. Research and probe deeper into having the surgery. They should involve other people aside from the Board of Directors and the Top Management; they should include the physicians, the employees/staff in the hospital and most importantly, the public. 4. They can dedicate a staff that will research and probe into having the CV surgery in their hospital and slowly invest in the program while improving other fields in their hospital. Recommendation:

From among our alternative courses of action, we think that dedicating a staff for studying and researching for having the CV surgery in the hospital and slowly investing in the program is highly recommended. They can slowly achieve a successful CV surgery program and not waste resources, and at the same time, they can also improve other facilities in their hospital. Lesson Learned: It is better to involve the general public and other members of the company in planning and implementing new programs. Who are the Stakeholders? Each of these stakeholder groups has its own expectations, and if the hospital executives are to gain their acceptance, they must set their performance goals to address the specific concerns of each group. The new Orthopedics & Neuroscience Unit compliments the dedicated operating rooms, state-of-the-art technology, surgical teams, orthopedic surgeons, neurosurgeons and neurologists at the hospital. What are Bloomington Hospitals Capabilities and Competencies? Over the years, Bloomington Hospital has improved their Heart and Vascular programs. They can now diagnose the cause of chest pain, to cardiac surgery, to cardiopulmonary rehab. We offer open-heart surgery, angioplasty, stent placement, and cardiac catheterization as well as several other state-of-the-art services. They have 64-slice CT scanner, Cardiac catheterization, Echocardiogram (ECHO), Electrocardiogram (EKG), Graded exercise test/Stress tests. Currently, they have 11 board-certified cardiologists and 13 board-certified cardiothoracic surgeons, as well as a team of nurses, technologists and other health professionals, care for patients and help them make an individualized plan for the treatment of their cardiovascular disease. Bloomington Hospital is a recipient of the 2006 Leadership Award for Clinical Excellence for achieving higher than 90 percent compliance on each quality indicator related to heart attack in the third quarter of 2005. Overall, they averaged 98 percent compliance with all indicators.
