the neurotransmitters in the brain Contradictions: Allergy, myasthenia gravis, glaucoma, breathing difficulties, severe liver disease, sleep apnea Interactions: Alcohol, anticonvulescents, antidepressants, antihistamines, antipsychotics, narcotics Side Effects: Dizziness, light headedness, anxiety, confusion, depression, irregular heartbeat, lack of memory, drowsiness, slurred speech Nursing Interventions: Dependence is known with benzodiazepines (a need to take regularly to prevent physical symptoms)- Severe withdrawal symptoms can occur such as seizures, agitation. Clomipramine (250mg/day) Class: TCA (tertiary amine) Usage: Obsessions and compulsions in OCD; obsessions or compulsions that cause marked distress, panic disorders, PMS Actions: inhibits presynapticmreuptake of neurotransmitters norepinephrine and serotonin; anticholinergic at CNS and peripheral receptors Contradictions: hypersensitivity to an trycyclic drug, acute recovery phase following MI, preexisting CV disorders, heart failure, angina, glaucoma, urinary retention, seizure disorders Interactions: Increased depression when used with alcohol, increased effect with cimetidine, disulfiram, omeprazole, contraceptives, decreased effect with theophylline Side Effects: mild drowsiness, sedation, depression, lethargy, apathy, fatigue, light headedness, disorientation, anger, hostility, CV collapse, constipation, diarrhea, dry mouth Nursing Interventions: monitor addiction patients, monitor liver function and blood counts in patients with long term use

Paliperidone (12mg/day) Class: Atypical Antipsychotic, Benzisoxaole Usage: Treatment of schizophrenia, acute and long term maintenance of schizophrenia Actions: Blocks dopamine and serotonin receptors in the brain and depresses the RAS. Antihistamine and alpha-adrenergic blocking activity may contribute to therapeutic effects Contradictions: hypersensitivity, lactation, dementia-related psychosis, pregnancy, history of arrhythmias, seizures, hypotension, diabetes Interactions: Risk of CNS depression when used with alcohol, risk of severe orthostatic hypotension If combined with other drugs that cause orthostatic hypotension Side Effects: headache, dizziness, akatheasia, somnolence, extrapyramidal disorders, tachycardia, tremor, parkinsonism, tardive dyskinesia, orthostatic hypotension, increased mortality in geriatric patients with dementia related psychosis, neuroleptic malignant syndrome Nursing Interventions: limit amount of drug given to patient with suicidal ideation, do not crush, cut or allow patient to chew tablet, normal to see tablets in the stool, monitor patients body

temperature to rule out infection, monitor for hyperglycemia, advise the use of contraceptives when on the drug

Paroxetine (75 mg/day) Class: Antidepressant Usage: major depressive disorder, OCD, panic disorders, social anxiety disorders, generalized anxiety disorder, hot flashes, diabetic neuropathy Actions: Potentiates serotonergic activity in the CNS Contradictions: use of MAOI or thioridazine, elderly, renal or hepatic impairment, lactation, suicidal patients Interactions: MAOIs, decreased effects of ohenytoin, digoxin, , increased serum levels and possible toxicity of procycidine, risk of serotonin syndrome if used with other SSRIs Side Effects: Somnolence, dizziness, insomnia, tremor, nervousness, headache, nausea, dry mouth, constipation, diarrhea, ejaculatory disorders, male genital disorders, Nursing Interventions: Administer once per day in the morning, shake suspension before using, ensure patient swallows tablet, limit amount given to suicidal patient, taper off drug

Risperidone (50mg/2weeks) Class: Antipsychotic, Benzisoxazole Usage: schizophrenia, short term treatment of manic episodes associated with bipolar 1 disorder, treatment of irritability associated with autistic episodes in children, treatment of OCD Actions: Blocks dopamine and serotonin receptors in the brain, depresses the RAS, anticholinergic , antihistamine and alpha adrenergic effects may contribute to therapeutic effects Contradictions: hypersensitivity, lactation, pregnancy Interactions: increased therapeutic and toxic effects with clozapine, decreased therapeutic effects with carbamazepine, decreased effects of levodopa Side Effects: insomnia, anxiety, agitation, headache, arrhythmias, nausea, vomiting, constipation Interventions: do not use in geritatric patient with dementia, maintain seizure precautions, monitor for signs and symptoms of diabetes mellitus
