Instructions to Offerors
1.1 Proposal Submittal
Proposal submittals shall be sent to CH2M HILL’s contract administrator Daena Mulleitner at
dmulleit@ch2m.com and/or via mail to CH2M HILL’s Denver office 9191 South Jamaica Street,
Englewood, CO, 80112-5946, USA. The offerer shall submit the following as part of the
proposal package:

• Preliminary Technical Approach: A description of the methodology and approach


required to complete each of the tasks identified in the SOW.

• Preliminary Project Schedule: A preliminary schedule indicating how the project


work will be sequenced and the total anticipated schedule duration. Subcontractor
shall submit a schedule as described in Section 7 of this Statement of Work. The
proposal shall present schedule information in the form of a project schedule using
critical path method (CPM) logic. Demonstrate how you would implement and
sequence project work at the listed sites in Indonesia. If awarded, this schedule shall
serve as the baseline construction by which CH2M HILL will coordinate the work
with the various stakeholders; any changes to the schedule shall be submitted to
CH2M HILL for approval.

• Organization Chart: Identify the site superintendents for each project, as well as any
lower-tier Subcontractors that will be used, and the proposed project management
team

• Qualifications and Past Experience: Technical qualifications must be demonstrated


through provision of the following:
⎯ A list of relevant projects performed over the past 5 years, particularly in
Indonesia. Identify successful completion of HA projects similar in nature to
those listed in the SOW, particularly in Indonesia. Identify your ability to
conduct project work simultaneously in Indonesia. Provide references for at least
five relevant projects.
⎯ Submission of relevant technical licenses for Indonesia (engineering,
architectural, electrical, mechanical, fire protection, general contractor) and/or
indicate strategy for how you will satisfy licensing requirements (e.g. using in-
country subcontractors).
⎯ Resumes for the PM and onsite Superintendents shall be provided to
demonstrate compliance with the experience requirements identified in Section
5.3, Supervision. The resumes shall demonstrate a minimum of 5 years
experience for each person managing and overseeing construction projects and

will optimally show previous experience in similar HA projects, particularly in
Indonesia. Onsite superintendents should have ability to communicate in
English.
⎯ OSHA 30 hour construction training must also be demonstrated for the Site
Superintendent. CH2M HILL will provide this training at its own expense at the
selected QC Subcontractor’s offices, provided the selected QC Subcontractor
covers expenses for its employees.

1.2 Selection Criteria


The following criteria will be used for best value Subcontractor selection, in order of
importance:

1) Experience in completing similar HA projects, particularly in Indonesia . Demonstrate


ability to conduct projects simultaneously in Indonesia in Central Java and/or Sulawesi,
and/or Banda Aceh, and experience of proposed personnel in accomplishing project work.
2) Technical Approach. Demonstrate how you will oversee design and construction work in
these remote regions of Indonesia with qualified personnel. Show a project sequencing that
is realistic yet allows simultaneous execution of multiple projects in Indonesia.
3) Cost. Cost estimate will be evaluated for accuracy and realism.

Humanitarian Projects in Indonesia
1.0 Introduction
CH2M HILL International Services (CHIS) has been requested by the US Army Corps of
Engineers, Pacific Ocean Division (POD) and Far East District (FED) to conduct design and
construction of Humanitarian Assistance (HA) projects in Indonesia. As part of this effort,
CHIS will procure one or more Quality Control (QC) Subcontractors on a firm fixed price basis
to provide QC support and inspections for these projects. The following projects are currently
anticipated to be completed by the selected QC Subcontractor:

It is permissible to propose to do work in one, two, or all three of the geographic areas (Central
Java, Sulawesi, Banda Aceh). Only one QC Subcontractor will be selected for each of the
geographic areas.
The QC Subcontractor will provide guidance and QC to the D-B Subcontractors during design,
and provide full time inspection support during construction by the D-B Subcontractors, with
CH2M HILL technical personnel periodically conducting QC inspections of overall project
work. This Statement of Work (SOW) defines the scope for the QC Subcontractor.
All contracts and payment will be in US dollars.

2.0 Subcontractor Responsibilities


The selected Subcontractor shall be responsible for:

• Preparing and submitting Statement of Qualifications, resumes for key personnel, and all
Health and Safety Plans, as required. All site superintendents conducting work at the site
must have OSHA 30 hour training, or be willing to attend such training at their expense
provided by CH2M HILL at the selected QC Subcontractor’s offices. Certificates of training
must be included in prework submittals if available.

• Preparing and submitting the necessary pre-construction and post-construction submittals


as described in Sections 5.0 and 6.0.

• Coordinating all inter-related work with D-B and other Subcontractors.

• Providing the necessary supervision, labor, and materials and any supporting
subcontractors to complete the scope of work as identified herein and in the project
drawings and specifications.

• Provide all labor, equipment, and materials to mobilize personnel and equipment to the site
at the beginning of the project and fully demobilize personnel, materials, and equipment
from the site at the end of the project. Subcontractor pricing will include all setup of storage
area and base of operations, acquisition of Personal Protective Equipment (PPE), and all
other tasks to initially prepare for the execution of work at these facilities.

• Performing all field activities (including associated quality control and testing) required to
complete the scope of work identified in this Statement of Work (SOW). The general
description of the work is provided below, but may not be all inclusive.

• Performing all field activities in accordance with US Army Corps of Engineers Health and
Safety manual (EM-385), as well as in compliance with all Host Nation laws, regulations,
and engineering and licensing requirements.

3.0 Scope of Services


This SOW encompasses the range of methods and technologies necessary to accomplish the
required QC support and inspection services in accordance with technical and regulatory

requirements of the US Army Corps of Engineers and the Government of Indonesia. The QC
Subcontractor shall perform the following tasks:

Pre-solicitation and Solicitation Support Services


The QC Subcontractor shall provide support to the CH2M HILL Project Manager (PM) during
D-B Subcontractor pre-solicitation and solicitation activities. This shall include, but not be
limited to:

− Providing advice to the CH2M HILL PM on construction implementation and project


packaging for the listed HA projects (e.g. packaging several proximally located projects
together for a single D-B solicitation).

− Identifying a minimum of three capable and experienced D-B Subcontractors for each
project or construction package. Demonstrate Subcontractor qualifications to the
CH2M HILL PM.

− Utilizing the project packages presented with this RFP (Attachment A), prepare
Schedule of Values (SOV), develop and translate conceptual designs that can adequately
serve as the basis for bidding by the nominated D-B Subcontractors, with review by the
CH2M HILL PM. Prepare a cost estimate for each project.

− Coordinating and conducting site walks at each project site, providing necessary
transportation for CH2M HILL personnel.

− Acting as the POC during solicitation activities, directing questions to the CH2M HILL
POC, and providing recommendations for responses.

− Providing any required support in obtaining local permits, establishing MOUs, and
providing support for governmental and stakeholder relations.

− Acting as POC with D-B Subcontractors for distribution of contract amendments, etc.,
and acting as the receipt point for submitted proposals.

− Evaluating submitted proposals and providing recommendations to CH2M HILL for D-


B Subcontractor selection based on a fair evaluation of Subcontractor proposals per the
selection criteria identified by CH2M HILL .

− Notifying D- B subcontractors of the selection results.

− Assisting CH2M HILL in getting contract documents in place, providing any necessary
translation, and facilitating signing of the D-B construction contract.

Design Phase Services
During the design phase and in coordination with the CH2M HILL PM, the QC Subcontractor
shall provide guidance and QC as the D-B Subcontractors prepare work plans and design
drawings and specifications for each project based on the conceptual project packages presented
in Attachment A. These services shall include, but not limited be to:

− Prepare QC Subcontractor Quality Control and H&S plans specific to the projects to be
designed and constructed. Prepare the QC plan in accordance with the CH2M HILL
Construction Quality plan attached to this RFP (Attachment B). Multiple projects may be
included in these plans, as long as site specific issues are properly documented.

− Coordinate a project kickoff meeting with the selected D-B Subcontractors in an


adequate facility, providing an agenda for the meeting that covers the key areas of
coordination necessary to properly implement the project(s). CH2M HILL personnel
may attend this meeting.

− Identify standard design drawings/strategies for facilities to be constructed and work


with the D-B Subcontractor to site-adapt these to existing project conditions. Ensure that
the D-B Subcontractors prepare all design drawings and specifications necessary for the
D-B Subcontractors to construct the facilities in a quality manner. Ensure that designs
are fully compliant with all local and host nation codes and regulations. Ensure that the
design is prepared to accommodate the budgeted construction costs for each project.

− As an alternative, the QC Subcontractor may propose to complete design of certain


projects using its own personnel and capabilities. If the offeror is proposing to do
this, the proposal should explicitly state which projects will be designed by the QC
Subcontractor.

− Conduct a QC review of the completed design plans and design drawings


/specifications and provide written review comments to the CH2M HILL PM.
Coordinate CH2M HILL review and approval of these documents. Ensure that all
CH2M HILL comments are appropriately incorporated into revised designs. In some
instances, the QC Subcontractor may be asked to coordinate and lead a design review
meeting with key stakeholders.

− Provide three copies of the approved, final design drawings and specifications to the
CH2M HILL PM.

− Work with the D-B Subcontractors to assist them to develop site specific Health and
Safety (H&S) and Quality Control plans that meet US Army Corps of Engineers
standards. The H&S plan shall include an Accident Prevention Plan and an Activities
Hazard Analysis. It is permissible to include multiple project sites in these plans if being
implemented by the same D-B Subcontractor, and if site specific issues are clearly
identified and documented.

− Provide design drawings in an electronic format approved by the CH2M HILL PM.

Construction Phase Services


The QC Subcontractor shall be responsible for monitoring the D-B Construction Subcontractor’s
performance during construction and ensure that established standards for timeliness, quality,
and cost are met in accordance with the final approved technical design and specifications. The
QC Subcontractor shall ensure the construction Subcontractor complies with the approved site
specific Quality Control Plan and the site-specific HSP and all safety requirements. Specific
duties shall include, but not be limited to:

− Assist and guide the D-B Subcontractor in preparing all required work plans, submittal
registers, testing plans for all project work. Provide these for review and comment to the
CH2M HILL PM. Ensure these plans are properly revised in response to CH2M HILL
comments.

− Assist and guide the D-B Subcontractor in preparing final H&S plan, APPs, and AHAs.
Provide these for review and comment to the CH2M HILL PM. Ensure these plans are
properly revised in response to CH2M HILL comments.

− Review all submittals and materials specifications and recommend acceptance or


rejection to CH2M HILL. Ensure that all required submittals are submitted by the D-B
Subcontractor.

− Coordinate with D-B Subcontractor during mobilization to the site, assuring that all
required planning, equipment, materials, etc. have been appropriately mobilized.
Conduct an Operational Readiness Review with the Subcontractor and CH2M HILL PM
to ensure readiness to begin construction work.

− Organize and lead a preconstruction meeting for each project, providing a suitable space
for the meeting. Prepare a meeting agenda and distribute to appropriate parties per
direction from the CH2M HILL PM.

− Monitor the construction Subcontractor’s progress and progress reporting. QC


Subcontractor shall review the Subcontractor’s submitted schedule for practicality,
adequacy in identifying work activities, and inclusion of approved changes. The QC
Subcontractor shall recommend approval or disapproval of the Subcontractors proposed
schedule to the CH2M HILL PM. If the Subcontractor falls behind schedule, all facts
concerning the progress and delays (excusable or controllable, laxity on the part of the
Subcontractor, estimated time due for extension of the contract period) and revised
estimate of the completion date shall be reported in writing to the CH2M HILL PM.

− Ensure that D-B Subcontractor’s activities are completely in accordance with the HSP
and AHAs at all times. Monitor D-B Subcontractor to ensure that daily tailgate meetings

are conducted to discuss the day’s activities and the potential H&S issues that may be
associated with those activities. Stop work of D-B Subcontractor if they are performing
in an unsafe manner or refuse to follow H&S plans and protocols.

− Provide full time inspection services during construction in accordance with the QC
Subcontractor’s QC plan and the CH2M HILL Construction Quality plan. Ensure that
the D-B Subcontractor fully implements the approved design and specifications, unless
deviations are approved by the CH2M HILL PM.

− Have access to a cellular phone and be available for discussions with the CH2M HILL
PM on a minimum semi-weekly basis.

− Prepare daily and weekly reports in a typed format per the QC plan. Provide
photographic documentation of key activities with each report. Submit reports to
CH2M HILL on a daily basis unless otherwise directed by the CH2M HILL PM.

− If unforeseen conditions arise, Requests for Information (RFIs) are submitted by the D-B
Subcontractor, or a Change Order is requested by the D- B Subcontractor, the QC
Subcontractor shall research the issue, provide a written description and
recommendation for resolution to the CH2M HILL PM, and track the issue until it is
fully resolved. Maintain a tracking log of all RFIs and Change Orders.

− Be continually on the alert for deficiencies in the D-B Subcontractor’s performance.


Immediate action is required when deficiencies are noted and the CH2M HILL PM shall
be notified if Subcontractor refuses to correct deficiencies. A complete record of the facts
relating to the deficiencies and efforts to correct them shall be made.

− Do not waive contract provisions, make or authorize changes to the contract design or
specifications, dictate the construction Subcontractor operations, allow the construction
Subcontractor to perform in an unsafe manner, or accept work not meeting contract
requirements without coordination with and specific written approval from CH2M
HILL PM.

− Immediately notify the CH2M HILL PM of any situation that may result in a contract
dispute, require a modification to the contract, or require issuing a stop work order to
the D-B Subcontractor. In any event, a complete record of the facts shall be developed
and submitted to the CH2M HILL PM.

− Provide input to CH2M HILL PM concerning the D-B Subcontractor’s performance at


the end of the project in a written and signed report. The report shall specify the
Subcontractor’s performance in maintaining schedule, quality and overall degree of
effectiveness.

− Submit accruals to the CH2M HILL PM by the 17th of each month that covers predicted
accruals for that complete month (1st to 30th). Present completion percentages based on
the SOVs or other format agreed upon with the CH2M HILL PM.

− Provide an update of the Subcontractor’s schedule and testing plan by the 25th of each
month.

− Review as-built drawings on a minimum weekly basis and provide final review of as-
builts prepared by the D-B Subcontractor at the conclusion of the project. Furnish the as-
builts for CH2M HILL review, and ensure that all comments are properly incorporated.
Ensure that any warranty, O&M, material or equipment specifications are submitted to
the CH2M HILL PM. Provide three copies of the final as-builts and other materials, with
one copy in an electronic format, preferably CAD.

− The QC Subcontractor shall review and verify all requests for progress payments to
ensure the request accurately represents work accomplished satisfactorily. The QC
Subcontractor shall complete and sign the pay request certification form that will be
forwarded to the CH2M HILL Contract Administrator (KA) for payment. The final
request for payment shall contain, in addition to a completed and signed pay request
form, a listing which depicts the final document submittals (i.e. as-builts, operation and
maintenance manuals, etc.) as well as the date of acceptance of these documents. The
listing shall be dated and signed by the construction Subcontractor and the QC
Subcontractor.

4.0 Additional Scope of Work Requirements


4.1 Site Preparation Activities
The QC Subcontractor shall oversee site preparation as necessary to prepare the sites for
implementing the work. The Subcontractor shall be responsible for ensuring the following
activities are conducted by the D-B subcontractor:

• Traffic controls

• Maintaining appropriate security (i.e., safety barricades with fencing and barricades, and
appropriate warning signage. Properly securing site at the end of each day.

• Keeping all work areas clean, neat, and maintained at all times.

• Stockpiles, roll-off containers, or other areas disturbed by construction operations.

• Protecting existing security fence/gates, structures, surface features, service operations and
the general site environment.

• Identifying and protecting all utilities, above and below ground, in the designated work
areas. Utilities shall be identified by reviewing appropriate drawings, conducting utility
toning, and performing site visits with appropriate Base utility representatives.

• Implementing a lock-out/tag-out program on all utilities to be cut to facilitate work
activities, including temporary diversion of storm water run-off. Care shall be taken to close
all upgradient valves on product supply lines prior to cutting and draining activities.

• Holding to a minimum any interruptions to existing utilities. The QC Subcontractor shall


ensure the D/B Subcontractor repairs/replaces within 3 calendar days any damaged
property resulting from construction activities.

4.2 Supervision
The QC Subcontractor shall identify a Project Manager (PM) to manage this project from project
award through project completion. The PM shall be the primary POC for correspondence
related to cost, schedule, and scope, as well as for providing all required submittals.
During construction, the QC Subcontractor shall maintain adequate on-site supervision at all
times when work is being performed. A designated Site Superintendent shall be identified for
each project site who shall be responsible for field execution and safety of all work being
conducted. The Site Superintendent shall have at least 2 years of previous experience with
similar construction projects, preferably with HA projects in Indonesia.
Resumes for the PM and Superintendent shall be provided with the proposal, and shall
document experience for each person managing and overseeing similar construction projects.
Personnel changes of the PM and Superintendent shall only be made upon prior approval from
CH2M HILL.

4.3 Waste Management


The QC Subcontractor shall be responsible for ensuring that the D-B Subcontractor completes
offsite disposal of all wastes generated on this project in accordance with host nation and local
regulations. The QC Subcontractor shall be responsible for tracking all wastes generated,
adequately characterizing them for disposal (as required), and maintaining records (trip tickets,
disposal certificates, and/or manifests) documenting waste management activities. Copies of
records shall be provided to the CH2M HILL Construction Manager and logged in the
Submittal Register.

4.4 Heath and Safety


The QC Subcontractor shall be responsible for the health and safety of their employees and for
overseeing the health and safety of the D-B contractor personnel. The QC Subcontractor will
designate one site employee as the "Site Safety Coordinator" (SSC) who will interface with the
CH2M Hill’s designated Construction Manager/Site Safety Officer in matters of site safety.
The QC Subcontractor shall submit an Accident Prevention Plan that satisfies the requirements
in CH2M HILL EM 385-1-1, November, 2003. The Accident Prevention Plan will be submitted
to CH2M HILL prior to the field mobilization for review by CH2M HILL Health and Safety
Manager. The Plan shall be reviewed and accepted by CH2M Hill prior to construction
activities or implementing associated activities. The QC Subcontractor shall comply with the
general requirements identified in both CH2M HILL’s Site Health and Safety Plan and in the
QC Subcontractor’s provided Accident Prevention Plan.
Activity Hazard Analyses (AHAs) will be required to be submitted by the QC Subcontractor
prior to mobilization, as described in Appendix F of the Request for Proposal (RFP). AHAs

shall be reviewed with all QC subcontractor personnel prior to implementing associated
activities. All AHAs submitted shall be reviewed and approved by CH2M Hill prior to
construction activities or implementing associated activities.
The QC subcontractor shall conduct a daily safety briefing based on the AHAs for the task(s) at
hand and a daily Pre-Task Safety Plan developed for those activities, and ensure the D-B
Subcontractor conducts a similar briefing for its personnel.
The QC subcontractor shall enforce safety rules on the job and be prepared to take action to
eliminate unsafe work practices and conditions.

4.5 Work Planning


A Work Plan will be submitted to CH2M HILL prior to construction supervision that describes work
activities and detailed technical approach required to complete each of the tasks identified in
the SOW. This includes efforts to minimize impacts to facility operations during construction.
The Work Plan must be submitted within 45 days of notice to proceed.

4.6 Quality Control (QC)


The QC Subcontractor shall prepare a Construction Quality Control Plan and perform field
testing in accordance with industry standards to ensure compliance with project specifications,
technical requirements, and manufacturer instructions. QC requirements will be further
identified in the Submittal Register that will be prepared with the D-B Contractor.
The Quality Control Plan may be included in the Work Plan and will address the following
items:

− Structure of the quality control organization and the roles/responsibilities of those


individuals on the project
− Description of the quality control program including meetings, procedures and
resolution of conflicts and change management
− Design control including calculations and as-built drawings
− Photographs
− Document control
− Contractor deliverables (including daily production, QC, and cost tracking reports)
− Measurements and equipment calibration
− Non-conformance reports
− Field log and documentation
− Data acquisition
− Reporting and QC Documentation requirements for the project
− Inspections/Observations to be performed on the project
− Testing to be performed on the project (including testing plan and log)
− Submittals to be performed on the project (including submittal register)
− Computer hardware and software (as applicable)
− Audits
− Continuous improvement procedures

Upon review, CH2M HILL may require revisions to these plans in order to ensure the project
objectives will be achieved in accordance with applicable rules and regulations and at an
appropriate level of worker safety. The QC subcontractor shall revise the documents until
acceptable to CH2M HILL.
A testing log will be maintained, and if needed, updated on a weekly basis to demonstrate all
testing is being performed in compliance with the Scope of Work and specification
requirements. Copies of the testing log will be provided to the CH2M HILL Project Manager.

4.7 Products and Materials


Prior to purchase, the QC Subcontractor will submit to CH2M HILL the product or material
details for approval on the appropriate CH2M HILL material submittal form. Products and
materials will not be purchased or installed without obtaining approval from CH2M HILL. A
schedule listing the delivery dates of materials will be submitted to CH2M HILL.

4.8 Changes in Scope / Technical Direction


Any work performed by the QC Subcontractor outside the approved scope of work without
CH2M HILL’s prior written acceptance shall not be considered for compensation.

4.8.1 Requests for Information


Requests for Information (RFIs) shall be used to communicate and document clarifications and
modifications requested by the D-B subcontractor. The RFIs will be tracked and logged by the
QC Subcontractor and submitted to the CH2M HILL Construction Manager and PM to ensure
that each RFI is fully addressed and that changes to the plans, drawings, and specifications are
completely and accurately documented.
The RFI process involves identifying the situation in the field that requires change. This is done
by the QC Subcontractor..
When a change is identified by the D-B or QC subcontractor, the following process will be
followed:

• QC Subcontractor reports the concern to the CH2M HILL CM or PM.

• The QC Subcontractor then prepares an internal memorandum, in the form of an RFI,


identifying the concern and forwards it to the CH2M HILL Project Manager. The RFI will
contain the TO number, an RFI Identification Number and a Title.

• The response should include a narrative explanation of the resolution and attach any
drawings or specifications required to complete the work.

• The response is returned to the Project Manager.


The RFIs are numbered sequentially for individual TOs and filed at the job site, with the CH2M
HILL Project Manager, and the CH2M HILL design team.
Changes to as-built drawings as a result of a RFI shall be identified with a symbol in the border
identifying the RFI identification number and title. The drawing should also be marked with a
cloud, circle, etc. to distinguish the change from the original drawings. The sheet will then
supercede the existing drawing in the drawing set.

Note: the RFI process is a field construction tool for documenting changed field conditions or other issues
that may require a deviation from project requirements identified in the drawings and specifications

4.8.2 Nonconformance Report


The CH2M HILL CM or QC Manager shall notify the QC subcontractor of any detected
noncompliance with the foregoing requirements. The QC subcontractor shall take immediate
corrective action after receipt of such notice. Such notice, when delivered to the QC
subcontractor at the work site, shall be deemed sufficient notification. If the QC subcontractor
fails or refuses to comply promptly, the CH2M HILL CM or QC Manager may issue an order
stopping all or part of the work until satisfactory corrective action has been taken.
Noncompliance notification or stop work orders will be documented in the daily report and a
Non-Conformance Report (NCR). Completion of corrective action will be noted on the daily
report. Verification of the corrective action and its results will be performed by the CH2M HILL
CM or QC Manager and documented in the daily report.

4.8.3 Resolution of Conflicts


If the CH2M HILL CM detects a nonconforming item, the issue shall be investigated by the
CH2M HILL QC Manager. If the CH2M HILL QC Manager determines that additional
corrective action is warranted, the CH2M HILL QC Manager will document and review the
issue with the QC Subcontractor Construction Manager/Site Superintendent and Project
Manager. The CH2M HILL CM and QC Manager have the authority to stop work on any
nonconforming activity.

4.8.4 Corrective Measure Plan


Resolution of failing test results or noncompliance reports shall be completed through a
corrective measure plan. The corrective measure plan shall be developed and documented by
the CH2M HILL CM, QC Manager, and Project Manager. The agreed upon corrective measure
plan will be implemented and documented by the CH2M HILL Manager. Completion of the
corrective measure plan is the responsibility of the CH2M HILL QC Manager.

5.0 Administrative Requirements


5.1 Scheduling and Planning
The Offer or is required to state in his Bid (1) the estimated duration of performing the various
activities described in this Scope of Work, including all submittals, and (2) the total estimated
duration of performing the total project. This information shall be provided in the form of a
project schedule using critical path method (CPM) logic. If awarded, this schedule will serve as
the basis for all scheduled construction activities; any changes to the schedule will be submitted
to CH2M HILL for approval. The final schedule must be closely coordinated with CH2M HILL
and base personnel to ensure minimal unanticipated disruptions to the users are experienced.

5.2 Project Meetings


The QC Subcontractor shall facilitate and attend meetings through progress of design and site
construction work. The project meetings shall include, but are not limited to, design review
meetings, a pre-construction meeting for each building, daily tailgate meetings, and weekly
progress meetings.

Meetings conducted will be recorded in Project Status Meeting minutes prepared by the
Subcontractor and submitted to the CH2M HILL PM. The QC Subcontractor PM and
Superintendent must review and approve all meeting minutes.

5.2.1 Pre-construction Meeting


Prior to any on-site construction work, the QC Subcontractor shall schedule a pre-construction
meeting to discuss the following subjects, as a minimum:
• Required schedules
• Status of Bonds and insurance (as applicable)
• Sequencing of work items
• Payment procedures
• Project changes and clarification procedures (including RFIs)
• Use of site, access, office and storage areas, security and temporary facilities
• Major product delivery and priorities
• D-B Subcontractor’s health & safety plan and representative (including required employee
records)
• D-B Subcontractors site safety and security plan and procedures
At a minimum, the PM and Superintendent shall be in attendance at the pre-construction
meeting.

5.2.2 Daily Tailgate Meetings


A daily tailgate meeting shall be held each morning at the site by the QC Subcontractor to
discuss the following subjects, as a minimum:
• The work planned for the day
• Changes in work assignment
• Health and safety issues
• Review problems encountered the previous day
• Sign the safety task assignment form prior to beginning any work onsite
The QC Subcontractor Superintendent, D-B Subcontractor Designated Safety Coordinator
(DSC), and all onsite workers, including lower-tier subcontractors, will be present at the daily
tailgate meetings.

5.2.3 Weekly Progress Meetings


The QC Subcontractor shall participate in regular progress meetings at the site, conducted
weekly, as required, to review construction progress, technical or regulatory issues, progress
schedule (including a two-week look-ahead schedule), and submissions schedule, application
for payment, subcontract modifications, and other matters needing discussion and resolution.
The following will be accomplished at each weekly progress meeting:
1) Review the minutes of the previous meeting.
2) Review the schedule of the following items:
a) Work or testing accomplished since last meeting
b) Rework items identified since last meeting
c) Rework items completed since last meeting

3) Review the status of the following submittals:
a) Submittals reviewed and approved since last meeting
b) Submittals required in the near future
4) Review the work to be accomplished in the next 2 weeks and the documentation required.
a) Establish completion dates for rework items.
b) Identify inspections required.
c) Identify testing required.
d) Establish the status of offsite work or testing.
e) Identify documentation required.
5) Resolve QC and production problems.
• Address items that may require revising the Construction Quality Plan, such as changes in
procedures.

5.3 Documentation and Submittals


The QC subcontractor will control and schedule all submittals for the D-B Subcontractor. These
submittals include all items listed in the contract document and listed or specified in the project
specifications, drawings, RFP, this Scope of Work, and the Submittal Register. A brief summary
of the required submittals are summarized but not limited to in the Submittal Register
The QC subcontractor shall log and track all submittals in the Submittal Register. Specific
responsibilities regarding submittals include:

• Coordinating all submittal actions

• Maintaining necessary submittal records in an organized fashion

• Maintaining and tracking submittals in the Submittal Register

• Reviewing and certifying all submittals for compliance with the project plans, drawings,
and specifications

• Clearly noting substitutions or variations of specified requirements.


Units of weights and measures used on all submittals will be consistent with those used in the
project documents.
The QC Subcontractor must ensure that the submittal packages are complete so that valuable
time is not wasted and effort lost on a re-submittal. Each submittal shall be reviewed for
completeness and compliance with contract and project requirements by the QC subcontractor
prior to submission to CH2M HILL. Submittals that do not comply with the requirements or
not approved will be returned to the QC Subcontractor for correction and re-submittal.
Certification of the approved submittals will be indicated by signing or initialing and dating the
submittal form by the CH2M HILL Construction Manager or Designee.
Submittals will be sent via email (and hardcopy if required) to the persons specified in the
Submittal Register. These persons may include:
• CH2M HILL Construction Manager, to be determined.
• CH2M HILL Contracts Administrator, to be determined.
• Mr. Jon Herwig, CH2M HILL Project Manager: jherwig@ch2m.com.
• CH2M HILL Project Engineer, to be determined

5.3.1 Daily and Weekly Submittals
At the completion of each workday the QC Subcontractor shall submit a Daily Report
documenting activities conducted throughout the day. The Daily Report shall be submitted to
the CM every week. At a minimum, the QC subcontractor shall include in their daily report the
following:

• Tailgate safety meeting minutes and signatures


• Project status meeting minutes
• Daily activities throughout the day
• Testing Plan and Log
• Permits
• Chain-of-Custody records (if applicable)
• Waste disposal documentation (if applicable)
• Proposed activities for the next working day
• Other submittals as required in the Submittal Register.

On a weekly basis (during weekly progress meetings), the QC Subcontractor shall provide a
summary of work accomplished, a two-week look ahead projection, and a revised construction
schedule reflecting any changes that have been identified during the week. This shall also
include copies of any test reports, material receiving reports/receipts/packing slips (as required
to verify the quality of the materials received), and waste disposal tickets (if applicable).

5.3.2 As-built Drawings


The QC Subcontractor shall be responsible, along with the D-B Subcontractor, for maintaining
as-built drawings of all project work. These shall be updated periodically and available for
inspection by CH2M HILL. At the conclusion of the project, the QC Subcontractor will provide
accurate as-built drawings in CAD and hard copy format for all project work.

6.0 Services Provided by CH2M HILL


CH2M HILL will provide support for the following services:

• Provide selected specifications and design drawings, as determined to be appropriate by


CH2M HILL.

• Provide periodic technical oversight of field activities.

• Coordinate, participate, and lead any discussions or clarifications required by the


Subcontractor with USACE personnel.

7.0 Schedule
The Subcontractor shall submit a construction schedule for performing the various activities
required to complete all work required.
QC Subcontractor support for D-B Subcontractor procurement will begin immediately upon
award, and must be completed within 2 months. The submittal of the Work Plan will be
required within 45 days of notice to proceed. Design work must be completed within 4 months
of NTP. CH2M HILL requires that all work be completed over a maximum 15 month period of

time. The Subcontractor shall notify CH2M HILL if this schedule is not acceptable, prior to
accepting the Subcontract. Exceptions to this schedule preference must be made as part of the
cost proposal. The final schedule must be closely coordinated with CH2M HILL to ensure
minimal unanticipated disruptions are experienced.
