Important Note: Passing in all grading periods and the Final examination means having a rating of seventy-five percent (75%), based on the Official Transmutation Table of the college. Examination Policy: The following rules and regulations will be strictly implemented: 1. No special quiz will be given to students who are absent or late. For long tests and major examinations, a letter should be presented together with the admission slip and / special permit in order to take the test/exam. 2. Cheating in any form is a major offense. Students caught cheating will be given a grade of 5.0 for the particular examination and will be referred to the Discipline Committee for sanction. Likewise, a deduction of 10 points in attitude will be strictly imposed. 3. Quizzes will be given after discussion of each topic 4. Long tests will be given every week. Expectations on Students: 1. Questions are entertained anytime. 2. Late students should not enter the classroom so as not to disturb the class. She/He can only enter the room after the break. 3. Active participation of students in class discussion is required. 10

4. Discipline inside the classroom should be strictly observed. A student who misbehaved inside the room will be sent to the Discipline Committee or the Guidance Office and will not be admitted in the class without a written permission from the said offices. Assignments/ Presentations: 1. Students will be required to submit reaction papers and other written assignments on time. No assignments or requirements will be accepted after the due date. 2. Take home examinations will be given every week and will be discussed in class on the first day of the following week. 3. Students may be assigned to report in class. OHP or PowerPoint presentation is encouraged. Written output should be submitted before reporting. Failure to report on assigned schedule means a grade of 5.0 for the said activity. 4. A portfolio (must include all things learned inside the classroom and applied in the clinical setting) must be submitted two days before the final examination.
