Progress Note Narrative: CW met with BM before her visit with WinonaCW provides BM with a list of referrals for mental health clinics and parenting programs- BM replies" you can give me that but I'm just going to give it to my lawyer" CW states this is what we are reccomending you do to reunify BM states" i allready have that list" CW states this is a list of mental health clinics as well as a list of parenting programs BM asks"whay would I need that" CW replies because you have not completed a parenting program and if you have you need to sign releases so agency can get the documentation BM's tone is very negative throughout this short conversation and CW isn't going to be cursed out again like last week CW tries to start to explain to BM that CW can help BM get into the programs if she is having trouble and CW has a form for BM to sign to document what we spoke about BM states "my daughter is outside please open the door!" CW opens the door and tells the 2 security waiting outside who hear BM's tone that this meeting is over now due to BM's non compliance- no need for security

CW informmed BM the form will be in file that she refused to sign but did receive the referrals and that she is non compliant with CW CW informs BM that if she uses foul language during her visit like last week CSW will immidiately end the visit and security is also made aware of this- This is as directed by AD Schwarz CW leaves

TODAY'S PROCEEDINGS: Respondent Winona Palmiotti complained about having no counsel and reported that her attorney is on vacation. Ms. Palmiotti complained about the subject child losing weight. Ms. Palmiotti complained about CW Lisa Scafide disclosing information about the child to the child's aunt without a release of information. ACA Frank Albert! noted that the respondent put forth the aunt as a resource for the child and the court directed DSS to explore the aunt as a resource, therefore the CW had to talk to the aunt about the child in order to comply with the court's directive. Upon arrival vehicle was GOA. PO continued to canvass area and at approx 1:30 did observe 2 vehicles engulfed in flames parked in the driveway at 318 Ellison Ave. PO notified CB and fire dept was dispatched, homes in area were vacated, while conducting investigation a red gas can with a wick was discovered in between both vehicles. 8-2-05 10:20am 318 Ellison Ave Westbury Assault 3rd Sub 1 photos taken Victim Piscitelli reports having a verbal argument with his girlfriend where she became violent. Progress Note Narrative: pc from a woman by the name of Dorothy Murphy. Ms Murphy claimed she is Winona's aunt. Winona's mother was her sister. She stated she understands cw can not provide her with any information but wants cw to know that if a witness is needed to testify on behalf of child Winona she wants to testify, her phone number is 973-248-8710. Ms Murphy advised cw of the following. "I was going to testify in upstate ny re: the abuse of Timmy. What she did to that child and my sister is unforgivable." "I blame her for my sisters death. She use to beat her. she abandoned both of them. That poor child was given horse medicine by Winona. When Winona wanted to knock him out. She would say it was easy to get over the counter. (??) Please do not give that little girl back. The only chance she has is to be away from her mother. I am 67 y/o I would take her myself but it would not fair to her." "I was talking to Winona right after you took the child. She was laughing saying she was contemplating coming up with a story that the father touched the little girl. She is not well." Progress Note Narrative: Csw went to daycare to pick Winona for visit with mother at the Mac office. Winona kept saying that she didn't want to go see mommy, she wanted to go home. Mom was in the lobby when we arrived. Winona's aunt, Tania Marzocco, was also there and asked to be in on the visit. Csw asked the aunt to wait until it's ok'd. Csw called the team and was advised she may come in. Mom was very upset that Winona's shirt was dirty. The day care said she was playing outside and got her shirt a little dirty. Mom was extremely agitated. Csw explained what happened. Mom raised her voice and said Winona wasn't suppose to be in day care or school but was suppose to have one on one care. Csw referred her to talk to the caseworker about her concerns. The aunt was viably upset and asked questions as where is Winona living, can I see her on the weekends, etc. Csw told the aunt that questions need to be addressed to the caseworker. The mom complained that Winona's shoes were too tight. Csw checked her feet and the shoe seemed fine. Mom just had one complaint after another and the aunt added and in turn encouraged mom. The aunt asked how do they know Winona is being properly taken care of, clothing and fed right. Winona complained that her teeth hurt. She pointed to the two back bottom teeth. Mom said she had a cap put on and maybe that is bothering her. Mom complained/They've done nothing for this child." The aunt left at this point to go to work. Mom gave Winona a shopping cart and bear from home. Mom used this to go over body parts with her. Mom also sat at the table with her and played a match game with her. They also pent time at the blackboard. When it was time to go Winona cried a little a didn't want to leave. Mom needed to be advise to just leave instead of hanging on to Winona. Once outside Winona stopped crying. Csw returned her to day care.

Progress Note Narrative: CW visited with FMMHflteend Winona in the foster home on a Notification of PlacementWhen CW arrived at FH, Winona greeted CW with a big hug FM and child had just arrived home when CW arrived Winona wanted to take off her pants and did so and proceeded to jump on the couch- FM told Winnona to put her pants back on and gave her a blanket and put Dora The Explorer on so CW and FM could talk FM reports that the Medicaid card has been straightened out, Winona attends full time daycare M-F 8:30-4:30 started on 9/29 She is eating good- she actually lost 5 pounds since placed in FC- now weights 52- Dr was glad to see this She is sleeping on and off she wakes up but for the most part sleeps through the night. Progress Note Narrative: pc from Barbara Grant. Ms Grant stated she has gotten at least 10 phone calls from Winona this morning alone. Winona is angry at her for not being in court today, "she wants me as a witness and she does not understand that I can do more harm then good. I know too much about her." "please don't tell her I said that, she can be very vindictive." cw advised Ms Grant that cw does not go back and discuss details of conversations with Ms Palmiotti. Ms Grant stated but she signed a release, cw advised that gives cw the ability to speak with her it does not mean that cw has to report back to Ms P what was discussed. Ms Grant advised that Winona knows that the "baby" is in Riverhead. "at court last week a social worker said she had to come from Riverhead." Ms Grant stated she does not know if Winona has attempted to find her in Riverhead. But Winona did say "you shop in Riverhead don't you. You can keep an eye out for her and let me know.'" Ms Grant advised that even if she did run into little Winona she would never approach her or the adult she was with regardless of where they where. "I would also never tell Winona that." "let me ask you Winona called me yest saying the baby was at the visit dirty, was she really dirty. I mean I understand if she came straight from daycare and didn't have time to change. I tried to explain that to Winona and all she could say was that she told you she did not want the child in daycare and you went ahead and put her in daycare." Do you know she wanted me to take my 5 kids out of school early and bring them to the visit. I told her absolutely not. What kind of mother would I look like then. Why would I take my children out of school so you can visit with your dau. She tells me your kids won't want to leave and that would be good, good for who winona? it would be funny, funny for who winona? she just doesn't understand. Progress Note Narrative: case conf held with Supervisor R Leto and AD Chris Allen, Cw advised domestics are from 1999thru 2005. The incident in August 2005 was where Winona cut Mr Piscitelli with glass. She was arrested. Both individuals were arrested in March 2000. First Winona was arrested for Criminal Mischief (driving over Mr P's lawn and pulling rain gutter off the home) at the time of her arrest she claimed Mr P physically assaulted her by pushing, shoving and kicking in the leg. She did require medical treatment at New Island Hospital in Bethpage. Mr P was arrested for assault. This was his only arrest. at this point in time there are no concerns pertaining to Paul Piscitelli. Should the DNA test confirm paternity we would support him as a resource for child Winona AD appraised of yest's visit, Winona's behavior, csw concerns during visit, mother's belief that child is in Riverhead as well of content of conversation with both aunts in regard to Winona "trying" to locate where this cw and cw Kevin Tase reside. AD advised cw Tase needs to be alerted. Although there is no direct threat security should be told and allow security to direct how to proceed. If a direct threat is made to either cw SCPD needs to be contacted immediately.

TODAY'S PROCEEDINGS: The case was calendared for Conference. At First Call, all parties were present and reported that the Respondent Mother, Non-Respondent Father and the child have all appeared for the DNA Marker Test. However, since the results are still not available, nothing further will be heard today regarding the P-Docket matter. At Second Call, Winona Palmiotti was sworn in and testified that she will not put her address on the record, as it is CONFIDENTIAL. The Respondent did, however, provide her address to the Court and to the Agency as: 1355 Locust Avenue in Bohemia, New York 11716. The Respondent's Attorney advised the Court that Winona Palmiotti is seeking unsupervised visitation now that she has established residency in New York. The Respondent's Attorney also supplied an Intake Appointment Letter dated October 5, 2009 from the Town Of Islip ACCESS Program The Assistant County Attorney objected to the Respondent being granted unsupervised visitation as she previously fled the first jurisdiction with the child after the Agency had obtained a Removal Order. The Assistant County Attorney apprised the Court that he attempted to contact the County Attorney in Ohio regarding the Respondent's case. However, the Assistant County Attorney informed that calls and messages remain unanswered and ttrere"tias"*een no response to date. - *..* The AWSfhey dor The Child had several concerns regarding Winona Piscitelli in that she was out of school for some time due to conjunctivitis but is now attending school again. The Attorney For The Child also reported that although the child is speech delayed, she has been slowly making some progress and is communicating with the teachers. The Attorney For The Child feels that the child should be evaluated for additional developmental delays. The Judge requested that the County expand the Respondent's visitation, The Assistant County Attorney consented to an increase to two (2) hours of supervised visitation per week. The Judge then addressed Winona Palmiotti and explained that the Court has great concerns for the child being that she fled another jurisdiction when threatened with the child's removal. The Judge notified the Respondent that establishing residency is not sufficient at this time and recommended that she begin attending her court mandated programs as suggested by the Agency. All parties then agreed to a six week adjournment date of November 13, 2009 in order to wait for the Paternity Test results.

Progress Note Narrative: pc from Winona asking if tomorrow's visit is going to be 2 hours cw advised cw has emailed csw who supervised visit if she can extend tomorrow by an hour if not visit may have to be rescheduled for another day. cw advised cw has not yet rec'd any documentation from ct worker showing judge approved 2 hours. Winona asked why she would lie. cw advised cw did say she was lying cw was just advising cw has not rec'd official documentation that the judge increased time for visit. Ms Palmiotti then went on telling cw she can not sign up for a parenting class at Family Service League cw has to refer her for preventive services. cw advised cw is not referring her for preventive services, a neglect petition has been filed so therefore there is need for protective services. preventive services were offered and she refused to sign the application. Ms Palmiotti then asked cw "expected" her to take any parenting classes if cw was not going to refer her. Cw advised cw gave her numerous referrals for a psych eval and just like cw told her in the past she needs to have a psych eval before she starts taking parenting classes, why. Cw explained that if there is a mental health diagnosis the doctor will make recommendations for treatment. If the doctor says go ahead with parenting classes then she can go ahead and sign up at one of the places cw gave her such as town of babylon town hall or town of smithtown or islip. Ms Palmiotti then started with her questions regarding Winona. How many children does she interact with on a daily basis? How many in general and how many are close to her. does she share food with anyone? does she sleep in a regular bed? or a toddler bed? at home she has a twin, how does she interact with the dog? does she feed the dog by hand?does she allow the dog to eat off her plate? or the table? how did she get pink eye? what time is her bed time, cw asked what time she would go to bed at night, winona replied 10pm sometimes 11. cw advised winona's bedtime is approx 8-8:30. cw reviewed everything foster mom had advised. given juice 1/2 water why? I never gave her watered down juice I only gave her gerber juice or Capri Sun Water. That does not have a lot of sugar cw advised does not like to drink milk I never had a problem giving her regular milk at night before bed in a sipee cup she has to pick the cup. I never really had junk food, she won't eat candy. does she bathe every day. yes. her hair is washed every other, why I would wash her hair every day. Ms P continued with her questions to the point that cw advised that cw understands she has a lot of questions however she can not expect cw to have all the answers cw does not live in the home with her child, cw will need to speak with fm to get her the answers to her questions and then she will advise. Progress Note Narrative: cw rec'd pc from csw Josephine that she just picked up Winona and child is crying she does not want to go. csw advised she is on her way but wanted cw to know that child is hysterical crying no mommy no home. during course of visit cw rec'd call that mother is requesting cw and supervisor come down to see child, mother sees a rash on child. cw went to public health nurse Gerry Sass and asked her to accompany cw to visiting room as mother believes child has a rash and mother wants it looked at. on way down to visiting room cw and public health nurse passed asst director Chris Allen cw advised of current situation and asked since supervisor was not available if she could also accompany to visiting. upon entering visiting room Ms Palmiotti asked cw where supervisor was. Cw advised she was coming. Public health nurse entered room and cw introduced, mother immediately instructed her daughter to come to her. "she has a scratch on her face." Winona did not pay her much attention then went over to her. mother began lifting her shirt in the back then the front stating she had a rash. The PHN viewed child's back and front and told Ms P there was no reason for concern. PHN advised she will document what she has seen today and Ms P's concerns. Ms P then stated her daughter has a fever. Can the phn take her temperature, phn advised thermametor is upstairs she will go get and come back down. cw waited in hall and allowed visit to continue, cw heard winona say she had to go peepee. her mother asked you have to go again? csw, mother and child went to restroom. phn returned while they were still in restroom. child came running on her toes back into room with mom following. mom stated she also has a black and blue on her leg. Winona eagerly went to phn who explained how the thermometer works, temperture was 98.7 and mom was advised of such, winona wanted her other ear done and phn stated she remembered when she went to see her at the home she wanted both ears then to. Ms P questioned why she went to the foster home, phn explained, winona then began questioning phn. phn did cut the conversation short advising she would document everything she discussed there was no concern for the rash and the black and blue along with the scratch were minor and probably result of child play. Her temperature was normal. Ms Palmiotti replied really her temperature is normal. Yes 98.7 she's normal. cw and phn left visiting room and visit continued. upon arrival back at desk cw was advised by AD Chris Allen that she was just informed that Ms Palmiotti placed a call to a Kevin Atkins in upstate new york stating her daughter is not getting services and she does not know what is expected of her. AD requested cw go back to visiting and ask Ms P to stop at reception after the visit and have cw contacted cw and AD will come down and sit with her to address any questions or concerns. cw went back to visiting. "Ms Palmiotti I understand you have contacted a Mr Atkins upstate. If you have any questions or concerns you want to discuss stop by visiting before you leave and ask them to call me. My Assistant Director and myself will come down and speak with you." Ms Palmiotti stared blankly at cw nodded her head slowly turning it to the left, "so what if I called Mr Atkins" cw advised well if you have questions or concerns we can speak with you about them so stop by the reception desk, no response Ms Palmiotti? ok? fine. cw then rec'd pc from Barbara Grant advising Ms P was sitting in the parking lot. the time was 4:1 1pm cw asked why she left the office when she was told that if she wanted to address her concerns cw and supervisor would meet with her. Ms Grant stated she would speak with her niece. 4:20pm cw rec'd pc from reception advising Ms P was in the lobby stating she has an appt. cw and asst director Chris Allen greeted Ms P in the lobby of building. Cw directed Ms P back to the conference rooms. Ms P stated no she did not wish to speak to anyone right now. she advised she was very angry at this cw. cw advised if she has any concerns she could speak to AD without cw present. AD advised she would give her the oppty to sit and speak about her concerns that she has re: this cw. Ms P refused. She stated she would make an appt. Ms P was repeatedly advised that cw and AD are willing to speak to her to address her concerns again she stated she was to angry to discuss. AD advised we can't address concerns if we do not know what they are. Ms P replied that is a good point. I will speak to Mr Adkins about my concerns. Ms P also stated she planned on letting the court know of her concerns.

Progress Note Narrative: Csw picked Winona up from day care for a visit with mom at the Mac office. As soon as she saw the van she stopped and said no mommy. Csw had to coach her to get into the van. She cried and screamed all the way to the office. Csw called the caseworker, left a message, to advise. Mom was in the lobby when we arrived. Mom gave her a hug upon seeing her. Once in the room mom asked about her blotchy face. Csw explained that she cried all the way to the office and that probably what the blotches were from, that she didn't have them when she was picked up. Nonna (as she refers herself as) kept pointing to her wrists and said "burn". Mom said yes, burn. The area was red. Csw called and left a message for the caseworker to advise. She kept redirecting and asked her about her face. Winona didn't say anything. Mom brought them lunchables. There was a peanut butter cup in it. Winona went to eat it. Mom asked her"Since when you eat chocolate?", and was said in a stern voice. Winona wanted to sit on the rocking horse. Mom took it out of the room and told her that she was too big for it. Winona wasn't. Through out the visit mom talked to her in a stern, almost combative voice. Winona then went to the doll house to play. Mom just sat on the couch and Winona brought the house to mom. She did this with the toys she wanted to play with as mom just sat on the couch. Mom complained that most of the toys are broken in the room. She also complained that the toys don't have batteries. She told Winona to complain to the state. She repeated this a few times. Winona brought a book to mom. Mom said to her, "Why this book, get another one.". Nothing seemed to please mom. Mom left during the visit to go to the bathroom, saying it was that time of the month. Winona said she needed to go after mom got back. We all went to the bathroom. Mom noticed a rash on her back and asked to speak to the caseworker and supervisor. Csw called up. Caseworker and the nurse cane down. The nurse too her temperature at mom's insistence. Temperature was normal. The nurse said the rash didn't look like anything to worry about and that she would make note of it. Before the end of the visit the caseworker came down and asked if she would like to meet after the visit as mom called the state. Mom just started at the worker and the worker needed to ask her again. Mom finally said ok. When it was time to go Winona started to cry and didn't want to go. Mom was feeding into her and csw finally had to ask mom to leave. Csw had to ask her several times. Progress Note Narrative: Interview at DSS: CW interviewed Father at DSS regarding the specifics of the incident between himself and Mother as documented by the police report from 8/2/05. Father stated that for the month leading up to child's birth, and in the following weeks, Mother was becoming disruptive and destructive in the household. Father stated that he was feeling a lot of pressure from her behavior and the mind games she was playing with him. NOTE: According to Father, mother was unfaithful to him during their relationship, and would constantly, throughout the pregnancy, state that the child may or may not be his, depending on her mood etc. Father stated that she would act in a mentally abusive way towards paternal grandmother, and in mentally and physically abusive ways towards him. Father stated that he even cried on one occasion, because it was very straining on him. pick up and leave with child. Father stated that in the past, she would leave for 2-3 months at a time, because she was a truck driver. Father stated that when she would return, she would constantly call him from home to the point where he was unable to have any social life. Father stated that on the day in question, mother was in the basement of the home, with the child about "15 feet away from her." Father stated that he went down in the basement and Mother began dropping glasses onto the floor. Father stated that he initially told her to stop and tried to speak civilly to her. Father stated that Mother continued to methodically drop glasses on the floor and the situation began to escalate. Father stated that he picked up some glass from the floor, cut himself on his forearm, and stated "I'm going to get you now, bitch." Father stated that he then called the police, and when they arrived, he told them that she cut him. Father was visibly upset during the interview and appeared remorseful of his actions. Father asked if this would affect his chances of becoming an intervener. CW told him he had to speak with the primary worker in Suffolk. Progress Note Narrative: would like to have own evaluation conducted by her own team she was advised that cw did not think that was possible becuase the child is not in her care but cw would speak to her supervisor and let her know, winona then stated she would like phone number for county atty Frank Albert!. sew placed winona on hold sew does not have phone number for cty atty and checked with another team supervisor and was instructed to tell Ms Palmiotti to contact Family court or the bar association. Winona wanted to know why sew could not provide number, sew advised sew does not have number and check with another supervisor who suggested she contact Family Court or Bar Association. Winona then wanted to know why cw did not have phone number. How is cw presenting case if cw has no telephone number to speak to cty atty. cw advised that is not how things are done. Winona again asked why sew said she could not have an independent team eval her dau. why is it sew deciding who evaluates her child, cw advised she already said Just Kids was ok with her and Just Kids is the agency who will be evaluating her dau. Winona stated so you are telling I don't have a say. I am telling you you don't have custody your dau is in the care and custody of the comissioner. You already said Just Kids was fine. I still have rights. I know you still have rights your rights have not been terminiated. I will speak with my supervisor, okay. I am going to let the court know okay I am going to let Spring Valley know. cw spoke with AD Christine Allen evaluation will still be conducted by Just Kids. Progress Note Narrative: On 10/14/09, Caseworker Lisa Scafide requested that I accompany her to the visitation room. The mother of Winona Piscetelli was requesting a nurse to assess the child's skin. Present in the room were the mother and child, the CSW, Ms. Scafide and myself. The child had a diffuse, lacy rash on the back of her neck and upper back. Significant Health Issues: Child Winona is 4 yrs and weight wise is off the growth chart Since entering foster care she has lost 5lbs and has received 4 immunizations and blood work. She had not been to a pediatrican since 2006. There are apparent delays in her speech and a full evaluation has been requested through Just Kids. Mother Winona does not have a mental health diagnosis however she is mandated to get a psych eval. Her behavior is very very concerning. She is extremely hostile. I have been cautioned by her relatives that she is very vindictive person. Reasons for DSS Involvement: Mother has an extensive history of 'evading' CPS dating back to 1989. In 2004 a neglect petition was filed in Sullivan County on behalf of her then 16 year old autistic and mildly retarded son Timothy on the grounds of abandonment. Child came into care as mother's whereabouts could not be ascertained. That child is unable to function independently in society and remains institutionalized in upstate NY> Mother is to have no contact with him. In 2005 she had another child a dau- Winona. Nassau Cty was going to petition against Ms Palmiotti and learned that there was an existing case in Sullivan County -Ms Palmiotti and child "disappeared" In April of this year Richland Cty Ohio rec'd a report on mother and child after child was brought into hospital with burns on hands and arms. She fell into a fire pit. Ohio was concerned with what they observed and offered Ms Palmiotti services. She agreed. Ohio requested an order to produce when they were unable to gain access to child. They then went into court to mandate services. Mom fleed again back to NY. A report was rec'd in NY when mother was seen at DSS center punching child. Mother and child were placed in shelter in Deer Park. Shelter had concerns and mother was offered preventive services by team 71. She fleed and went back to Ohio. Ohio sought to remove child and mother fleed again back to NY. She was staying at the Sunrise Motel. Child was removed on 9-16-09 and placed into protective custody. Current Presenting Problems: Mother is still not addressing her mental health issues and is extremely difficult to work with... Current petition does not have information about previous child We were instructed we need the court documents from upstate. Once I receive documentation from Sullivan County I will be submitting an amended lit sheet. Progress Note Narrative: CW met with BM after her visit with BMThis was first meeting with BM as this CW was just assigned this case on Tuesday The first thing BM said to CW was if she could show CW her daughter's arm- CW looks and sees some small bumps- BM goes on to say that this must be kept covered and lotion must be applied to it constantly and "the first 3 ingrediants of the lotion must not be alcohol, mineral oi! Progress Note Narrative: At the request of CW Lori Towns in examined Winona Pisciteli in the presence of her mother and Ms Towns. BM was concerned with a scratch on Winona's ankle. The scratch was about 1 X 2 cms in size and scabbed over. The scab was intact with new pink skin on the edges. It was not painful to touch. There was no redness, swelling, or discharge noted. I assured the BM there was no infection and it was healing well. BM then announced that the shoes that Winona was wearing were too small and were rubbing her ankle. She had bought her new sneakers in a larger size and put them on her. She put the old sneakers in a box and stated she would get rid of them. Ms Towns told her that they were not hers to discard and told her she had to give them back which she eventually did. Winona also had a runny nose and very pink chaffed cheeks with the color covering both cheeks and her upper lip. There was a burn scar on her right outer hand. BM was concerned about some dry skin in that area and stated that alcohol based hand cleaners burn her skin. She was assured that the FM was aware of this but that we would make sure the school was also aware.

Progress Note Narrative: CW met with BF, Paul Piscitelli at his home in WestburyBF shows CW the beautiful room he has set up for Winona in the hopes that he gets custody of her- It is pink walls, new windows with safety locks, new white furniture set with colorful flowers on it, tinker bell poster on the wall, flowers trimming the room, matching curtains, clothes in the closet, toys on the floor BF has done a wonderful job BF shows CW the rest of his home- he resides with his elderly mother who doesn't speak English- the home is hers but will be his onedayOn main level of home is child's room, BF's room, living room (where his mother sleeps on a futon- BF reports his father died in 2005 and his mother now can't sleep in the room they shared together) and Kitchen- all rooms are neat and clean Downstairs is a basement- used for storage and laundry Upstairs is a apartment that BF used to reside in but he will now move downstairs to be closer to child- Apartment is a one bedroom, small kitchenette and living room BF tells CW his story of events of history with BM and childBF reports he has been in a relationship on and off with BM since 1998- BM has always been violent towards him- for example she stabbed him in the face with keys and was charged with a class C felony, she hit him over head with a 2x4 BF doesn't know of any diagnosed mental illness for BM- while with him she never had any mental health treatment BM left NY on 5/19/06 at 12:30am (baby was around 10 months old)- she had received a $12,000 settlement from her mother's estate- BM would call him periodically and be in all different states- BF has recorded calls from BM In Oct of 2006 BM and child came back to NY- she got an apartment in Brentwood- it was filled with cockroaches- BF helped BM put in an airconditioner, etc, BM wanted a relationship with BF- BF didn't want this. BM left again in Nov 06- he couldn't find her In 2007 Paul states he registered with the putative father registry ON 9/4/2009- Barbara Grant (BM's aunt) called Paul and told him BM and child were back in Ny Then BM called him from Mineola tran station and said her car broke down and she had no money- BF went to pick them up- BF reports child greated him with a big hug They stayed with him in his home that night Then he put them up in a hotel- Meadowbrook Motor lodge for about a week and a half BM then realized that CPS was aware she was here so she wanted to run again- BF states he coopersted with CPS BM then went to stay a Relax Inn in Suffolk County All of a sudden BM's attitude towards BF changed and she was mean to him, wouldn't allow him with child, she kept calling him non stop asking what he was doing Then child was removed at 9-18 BM blamed him BF states that he witnessed BM spank child on her hand at least 5 times and childs hits her BM back BM pulled child's arm very hard BM's former boyfriend in Indiana, Kenny also witnessed BM pulling child's haif and pulling her over the car- he is willing to come forward and comply with CPS if needed BF reports he has family support from his siblings to assist in Winnona's care- he has allready went to local SD to inquire about CPSE services for child and the security in the school He works 1-8 but can switch this is necessary for child and his sister can also help with childcare Medical- will apply for Child Health Plus BF's healthy eating plan for child provided to CW CW tells BF that Judge may only give him temporary cusutody first and agency will supervise child or he can get permament custody and agency won't supervise BF does't care what he gets as long as he gets his daughter back CW reccommends BF use an agency such as EAC to do supervised visits with BM since she is a flight risk BF shouls also request an OP and if BM comes to him home he should call the police

Progress Note Narrative: Csw picked Winona up from daycare for visit with mom at the Mac office. Winona greeted mom with a hug when she saw her. Mom had a very bad stale cigarette odor. Mom brought a snack for Winona which she ate at the table. While looking at a book Winona said "daddy home". Mom gave her a stern look and said "no" very forcibly. Winona had a scared looked and quickly looked down. The caseworker came down to give mom court papers. After the caseworker left mom said the caseworker needs to come back as there are words that are blackened out. Progress Note Narrative: Csw picked Winona up early from daycare so cps worker T. Pisano could talk to her. Mom was in the lobby when we were ready for the visit. Mom finished a text message on her cell phone before greeting Winona. As soon as we were in the room mom complained that Winona was wearing nail polish. She stated she doesn't want her to wear it. Csw told her that she would need to talk to the caseworker. Winona told mom she has a booboo on her finger. Mom asked her how she got it and Winona responded she pushed in school. Mom told her it's bad to push people. Mom gave her applesauce and a drink. Mom then read a book. Winona complained about her foot. Mom took off her shoe and there was a mark from the shoe. Mom started to ask Winona if she wanted grandpa to go to court to get her. Csw stopped her before she said anymore more and informed her that was appropriate. Mom said she feels like she's being treated as a prisoner. Csw informed her visit standards are the same for everyone. About half an hour before the end of the visit Winona said she wanted to go back to school. Mom became annoyed and asked her why. Winona just shrugged her shoulders. Winona needed to go to the bathroom twice during this visit. Mom noticed that her underwear was inside out and started to complain. Csw asked Winona if she dressed herself today and she said yes. Csw told mom that would explain it. Mom also noticed a small bruise on her. During the visits mom does more time examining Winona and complaining then she interacts with her. Mom gave her a hug when it was time to leave. She tried to prolong it by calling Winona back to tell her something but Winona just kept walking. Csw returned her to daycare.

Progress Note Narrative: PC from MGM Carol Marzocco 518-280-5124- (BM's father's wife) She reports that she just found out on Friday night that they had a grandaughter, that she was in FC and they are interested in taking custody of her They found this out when they received a call from their other daughter, they called BM and she is in agreement to them taking custody- BM emailed them all her court paerwork They report they reside in Schectedy NY have room for the child but not the BM- they have a huge family with 12 grandchildren and support for allThey would like custody before Christmas CW explains to Carol that that child has a BF who has filed for custody of child and agency is in support of this-case on for trial tommorow and hopefully child will be placed with BF tommorow CW states if not then they are the next in line and this agency will have to request their agency CPS to do a courtesy HV and background checks, etc TheY would also have to come to Suffolk to file for custody of child Carol reports she and her husband were planning to come tommorow to court- CW states this is fine but they may want to wait until after tommorw if child doesn't get placed in her father's custody Carol states they will come tommorow and see what happens so they can file paperwork if she doesn't go with her father

Progress Note Narrative: CW picked up child from school and transported her to DSS for her 2 hour supervised visit with BMSince usual CSW who covers this visit was out sick today in order not to cancel the visit this CW picked up the child and supervised the first half of the visit and CSW mADELYN aDLIN supervised the rest of visit and transported child back CW arrived at DSS with child and BM was waiting in the waiting room for about a half hour due to the normal CSW being out and CW having to cover visit last minute- BM had McDonald's for her and child Security officer was standing with BM and told BM that when they get inside to the visiting room her McDondalds can be heated up BM and child pick a room to have the visit CW tells BM that when someone else comes in the room CW will be able to heat up the food- CW finds out the microwave has been moved into one of the visiting rooms- CW tells BM and child to come with her to the other room to heat up the food- Microwave is unplugged and CW cant get the plug in CW asks a CSW to assist CW- BM steps in a plugs the microwave in- BM has an additude and says to CW, "it is called a child proff you push it and it plugs in, wow that's sad that you don't know what child proffing is" CW tells BM that she should tell her concerns to the Judge and BM should really worry about her and her child because CW has custody of her child BM begins to yell and curse at CW causing the Security to come into the room and tell BM to calm down BM states "She started it!" CW takes BM and child back into the visiting room for visit to beginCW tells BM she really needs to start to cooperate with CW if she wants to reunify with her daughter BM yells," I don't know what the hell you are talking about", "Why don't you just sit there and shut the fuck up" CW tells BM that CW will end her visit right now- she is not to speak that way in frount of her child and BM has 5 seconds to improve her tone or visit is over CW goes on to say that BM's visit would have been cancelled today but CW didn't want this to happen so CW took time out of CWs day so BM could see her child so she needs to cooperate Security comes in the room again and tells BM she needs to cal down again and asks CW if she is going to be ok with this visit- CW states CW is fine and is only Supervising half the visit BM then proceeds to stare down CW in an evil fashion for a few minutes straight CW tells BM she should really stop staring at CW and pay attention to her daughter since this is the only time she gets with her BM does so BM and child eat their McDonalds and play with a small Yo Yo child brought with her Then child needs to go to the bathroom- CW accompanies them BM asks CW to come into bathroom stall with child and look at her bottom and legs- CW observes a red mark on child's bottom that appeeared to be from sitting on the toilet bowl (child was on the toilet bowl for about 5 minutes trying to make #2) BM then points out a very faint bruise on child's leg- This bruise appeared top be old and fading Child tern points out that she has a scrape on her knee- (very small) Child was unable to tell where she got it CW did not observe anything on the child to be out of the ordinary and children do get scrapes, burises and cuts during the day while playing CSW arrives to take over the visit BM allows child to play in the sink with the water and soap and fills it with bubbles from the soap- BM tells CW that it is recommended for child's burn for her to soak her hand in baby shampoo- CW will inform FM When back in the room and CW is about to leave- BM calmly asks CW to bring her releases to sign- CW does so and BM only signs for Access and asks for a copy of the release- copy provided- BM then tells CWthat she wants release to be ammended to not include all recommendation of her mental health evaluation- CW explains that this will then not be admitted in court since the evaluator should have had info from CPS prior to her going there and agency needs to know the recommendations- CW explians if they recommend therapy or not BM doesn't say anything BM states she doesn't remember where else she has attended or is attending- she states something is St Charles Hsp but she wont sign a release for this since there is other medical record there that she refuses to release to agency BM asks CW if her visits can be changed to the evenings on Fridays or Saturdays- CW states the transportation unit is currently going through changes and all new requests may take up to 3 weeks or more to be filled even if they have availability so CW will submit request but we will keep visit as is for now CW explains to BM that it will not be on a Saturday because the child will be with her father for the weekends- BM states agency will allow child to go with someone who is positive for Hep C- CW states yes you have allready amde that report to the State Central Registry and it has been unfounded and if you continue to make false reports to THE SCR you can be criminally charged CW tells BM she really needs to start to comply with agency because this is getting out of hand and she is making CW crazy!

Progress Note Narrative: PC to BF- he begins to explain the incident with BM on 8/2/2005 that he already discussed with Naussau CPS CW Wakefeild- BF states that the baby was 1 month old- BM sent him out to the food store with a shopping list, BF states he purchased the wrong cereal- BM was very angry and threw it out all over the backyard and was screaming For 2 weeks after the incident BM was putting him threw hell in his own home- On this day BM kept breaking things in the home (cups and picture frames) (baby was sleeping at the time) BF felt under deress and intimidated by BM- BF states he cut himself with the broken items and called the police and said BM did it (bARBARA Grant was on the phone with BM during this incident) BF wasn't arrested and BM was arrested BF reports he sufffered years of abuse by BM BF reports he had no idea where BM and baby were from 5/19/06 until Sept 2009

Progress Note Narrative: PC from MGM Carol Marzocco- she reports that she and Phil plan to attend the SPR on Friday and have information on BF (that he was high at last court date and that he has several indicated CPS reports) CW states the SPR is not a time to bring these items up- this is a meeting strictly for the mother to be informed what she needs to do in order to reunify with her child and that FC is time limited- CW doesn't think this is a good time for her to bring these items up- CW checks with Sup O'Donnell who states that no this isn't to be discussed at the SPR and that they should not come on that day if this is why they want to come- CW offers to meet with Carol and Phil on a later date to discuss there concerns for the child and offers a visit with child as well- Carol agrees Carol provides CW the SCR information she states BF has indicated reports in Sullivan County CW informs Carol that BF has no other children and never resided in Sullivan County- Carol reports the notices were mailed to his address in Naussau and he was indicated for "child abuse and maltreatment" Carol goes on to say that BF has indicated CPS reports in Suffolk and Naussau County- CW informs Carol that we are Suffolk County CPS and thisis untrue and we are working closely with Naussau County CPS on this case Carol states she has hired her own atty and"there is no way they are allowing their granddaughter to go with this man!" CW suggests Carol provide all this information to her atty so he can present this to the Judge and as for the meeting this will not be discussed at the time CW takes down the informationCarol reports Sullivan County has indicated reports on BF dated: 8/2/05, 10/17/05, 11/21/05, and 1/5/06
