Minutes of the Meeting January 9, 2012 Program: Arrival and first meeting with Foster Parents and Clinical

Details of the Meeting: During the first day, we arrived at Batas Silang Cavite at around 12 noon. We first went to our foster parent s house to meet them and to fix our thing. After that we went to the basketball court for our meeting with our clinical instructors, Sir Javier, Dr. Cajucom, and Dr. Aricheta. After that we went around the Barangay for a tour. After the tour we went back to our foster parent s house to know them and rest. Agenda y Meet with the Foster Parents and Clinical Instructors y Remarks The students were able to know their specific foster parents where in they will stay . The students had given a chance to establish rapport to their respective family and was able to meet their clinical instructor who will serve as their superior throughout their stay. The students were oriented on the tasks needed to be done, and the proper dequorum needed to be observed. The student were given a chance to assess the whole community for them to perceive the possible health threat that the community may possess.

Minutes of the Meeting January 9, 2012 Program: Discussion Where: Mr. Javier s Residence Time: 5:30pm -7:30pm Objective: To know our requirements and to learn more about the community. Person/s Involved: y y Sir Javier, Dr. Cajucom, Dr. Aricheta Sudents from group CA 10/11/12

Details of the Meeting: At around five thirty in the afternoon, we had a meeting in our clinical instructor s house. We had a bible reading and a short sharing. After that Sir Javier gave us the list of requirements, discuss about the community and gave our assignments.

Remarks The students were able to know the essence of understanding the Gospel reading and how it s correlation with their daily living The student were oriented regarding the importance of community assessment and were able to gained additional knowledge regarding the community. The Student were given the list of requirements that they should carry out before the end of their rotation Assignments such as johari s window is given for the student to be able to acknowledged their real personality .

Minutes of the Meeting January 10, 2012 Program: Election of officers/Discussion Where: Residence of Sir Javier Time: 9:00 am- 12:00 pm Objective: to be able to nominate possible leaders of our group. Person/s Involved: y y Sir Javier, Dr. Cajucom, Dr. Aricheta Sudents from group CA 10/11/12

Details of the Meeting: The day started with an opening prayer followed by a bible reading then the requirements/assignments of the students were submitted. A slight discussion by Dr. Aricheta was conducted followed by the election of officers and the signing of committee group leaders and members. In the afternoon ach committee group did their assigned tasks for the day and by the end of the day a post conference was conducted by the core officers held at the plaza.

were able to nominate and elect suitable student leaders for the said positions each student had given a chance to choose what committee they want.

Minutes of the Meeting January 11, 2012 Program: Introduction of Barangay Captain and Student Committe Where: Barangay Hall Time: 8:00am -9:30am Objective: To introduced ourselves to the barangay officials. Person/s Involved: y y y Hon. Benigno B. Cortez Sir Javier, Dr. Cajucom, Dr. Aricheta Sudents from group CA 10/11/12

Details of the Meeting: The day started with an opening prayer followed by Sir Javier opening remarks. Then the Barangay Officials and students committee were introduced. Then the baragay captain Hon. Benigno B. Cortez had a speech for the whole group after the general assembly and the said courtesy call. The courtesy call was a success and establishment of rapport was accomplished and recognition of each barangay official was acknowledged.

Minutes of the Meeting January 11, 2012 Program: Discussion of the Development of each committee in relation to their planned activities. Where: Barangay Hall Time: 9:30am-10:05 Objective: To be able to have a feedback regarding the development of the activities per committee. Details of the Meeting: The elected leaders per each committee were requested to share their progress regarding the different areas in which they will conduct their program.The Health Care Provider leader Jake Magcumot discussed about their successful courtesy call with the health center personnel s and was able to get the schedule of the availability of the health center for the community people. The SHN committee leader Justin Robles discussed about the problem they encountered in the school area .She discussed that the school requested their group to collaborate with the district supervisor Mrs. Alvarez for them to know if they ll be allowed to conduct their program in the said area. Then the group leader of the ACST Edgar Mirasol talks about the development of their group as he mentioned they are still trying to contact Ma am Kate for them to be able to know the list of available activities that will serve as a guide in choosing what activity they must prioritized .last but not the least is the AR leader Adrian Padecio who had given the whole group a simple but intellectual discussion of what did Dr. Aricheta thought them for them to be able to express themselves in the most effective way. . Remarks The leaders were able to discuss the development of their activities and the problems that they encounter in achieving their objectives.
