Up to two reports are required from referees who are able to evaluate your academic ability and research potential. Please provide your referees with the Academic RefereesConfidential Report form available at www.victoria.ac.nz/fgr/prospective-phds/publications/academic_referees_confidential_report.pdf If your prerequisite tertiary qualification was gained at Victoria University of Wellington, provide one report. If your prerequisite tertiary qualification was gained at a university other than Victoria, please provide two reports. One of the reports must be from your previous university. Advise your referees of the relevant closing date for your scholarship application (1 March, 1 July or 1 November). Reports must be sent directly to the Scholarships Office (address above) or attached to the application in a sealed envelope. Reports may be emailed to scholarships-office@vuw.ac.nz provided the referees use an institutional email address.

I consent to: (i) The disclosure of the personal information given on this form to recipients within the University, for purposes related to my application for a scholarship and as required by protocols between the University and external agencies, including selection committees. (ii) The Scholarships Office obtaining any information about me held by Victoria University of Wellington, and any other institute which I have attended, relating to my application for a scholarship.

(iii) The publication of my name and details of any scholarship which I may be awarded, together with any optional personal information which I may provide for this purpose. (iv) The disclosure of relevant information to sponsors of such scholarships. The Scholarships Office undertakes to store your application in a secure place in the event that you are successful in gaining an award or are selected as a reserve candidate for an award, and to destroy your application to preserve its confidentiality in the event that you are unsuccessful in gaining an award. Should you have reason to believe that information held about you in your application is incorrect, you have the right of access to, and correction of, that information.
