ASSESSMENT

Subjective: Masakit ang balakang at mga kasu-kasuan ko as verbalized. Objectives: With joints pain; reported. Pain scale rating of 4/10. With limited body movements; noted. With laboratory result of Uric acid- 385 mmol/L (normal178mmol/L345mmol/L)

DIAGNOSIS
Chronic pain related to inflamed joints secondary to excessive amount of uric acid in the blood.

PLANNING
After 4 hours of nursing interventions, the clients pain will be lessened.

INTERVENTIONS
Bedside and morning care rendered. Placed client in her most comfortable position. Health teachings imparted as follows: Use relaxation techniques like deep breathing/focus breathing. Use diversional activities like reading newspaper or chatting to relatives. Increase oral fluid intake at-least 10-12 glasses of water a day. Avoid fatty, salty, and uric acid rich foods like beans, red meats, glandular organs, and small fishes. COLABORATIVE: Administer celecoxib 200 mg once a day.

RATIONALE
To provide comfort. To provide comfort and relaxation. To reduce tension.

EVALUATION
After 4 hours of nursing interventions, the clients pain was lessened from pain scale rating of 4/10 to 2/10.

To divert attention to pain. To facilitate excretion of waste product like uric acid. Help lessen the amount of uric acid in the blood.

To relieve joint pain.

ASSESSMENT
Subjective: Ok na ako, wala na akong nararamdaman as verbalized. Objectives: No incidence of joint pain reported. Seems relaxed and comfortable. Performs ADL with minimal assistance needed and w/out feeling of pain upon movements.

DIAGNOSIS
Readiness for enhanced therapeutic regimen management.

PLANNING
After 2 hours of nursing interventions the client will be able to understand the health teachings imparted and will assume responsibility for managing treatment regimen and preventive measures to avoid progression of illness and sequelae.

INTERVENTIONS
Bedside care rendered. Assisted client on her most comfortable position. Health teachings imparted as follows: Increase oral fluid intake at-least 10-12 glasses a day. Avoid fatty, salty, and uric acid rich foods like glandular organs, beans, and small fishes. Eat foods rich in iron, fiber and Vit. C like green leafy vegetables. Have adequate rest and sleep.

RATIONALE
To provide comfort. To provide comfort and relaxation.

EVALUATION
After 2 hours of nursing interventions the client understand health teachings imparted and verbalized naiintindihan ko at gagawin ko lahat ng sinasabi mo.

To facilitate excretion of waste product like uric acid. Help lessen the amount of uric acid in the blood. To boost immune system and for proper absorption of iron. To prevent fatigue.
