DRUG STUDY
NAME OF DRUG GENERIC NAME: CLASSIFICATION THERAPEUTIC: Analgesic, Antipyretic, Antiinflammatory PHARMACOLOGIC: Central Nervous System Agent, Cyclooxygenase 2 Inhibitor, NSAID PREGNANCY CATEGORY: C (first and second trimester), D (third trimester) MECHANISM OF ACTION NSAID that exhibits analgesic, antiinflammatory and antipyretic action. It inhibits prostaglandin synthesis by inhibiting cyclooxygenase-2, but doesnt inhibit cyclooxygenase-1. Pharmacokinetics: Peak: 3 hours. Distribution: 97 % protein bound; crosses placenta. Metabolism: Metabolized in liver by cytochrome P450 2C9 enzymes. Elimination: 57% are primarily excreted in feces and 27% excreted in urine. Half-Life: 11.2 hours. INDICATION INDICATIVE FOR: 1. Relief the signs of rheumatoid arthritis and osteoarthritis. 2. Treatment of acute pain and dysmenorrhea, 3. Reduction of polyp formation in Familial Adenomatous Polyposis (FAP). CONTRAINDICATION CONCENTRATIONS: Contraindicated in patients with: 1. Hypersensitivity to colecoxib 2. severe hepatic impairment 3. asthmatic patient with aspirin triad 4. anemia 5. concurrent use of diuretics and ACE inhibitors 6. advanced renal disease 7. pregnancy in third trimester 8. lactation PRECAUTIONS: Use cautiously in patients who/with: 1. weight less than 50 kg ADVERSE EFFECTS MOST COMMON: Body as a whole: Peripheral edema and back pain. GI: diarrhea, abdominal pain, dyspepsia, nausea and flatulence. CNS: Headache, insomnia and dizziness. Respiratory: Rhinitis, sinusitis and pharyngitis, upper respiratory infection. Skin: Rash NURSING RESPONSIBILITIES BEFORE: 1. Assess patients range of motion, degree of swelling, and pain in affected joints before and periodically throughout therapy 2. Assess patient for allergy to sulfonamides, aspirin, or NSAIDS. Patients with these allergies should not receive celecoxib 3. Monitor fluid retention and edema especially in those with a history of high blood pressure or CHF 4. Monitor closely level of lithium when two drugs are given concurrently. 5. Lab test: periodically monitor Hct and Hgb, BUN, creatinine, and serum electrolytes. DURING: 1. Instruct patient to take celecoxib exactly as directed. Do not take more than prescribed dose. Increasing doses does not appear to increase effectiveness 2. May be administered without regard to meals. 3. Encourage bed rest 4. Instruct pt./SO to have small frequent feeding AFTER:

Advise patient to notify health care professional promptly if signs or symptoms of GI toxicity (abdominal pain, black stools), skin rash, unexplained weight gain, edema occurs 2. Patient should discontinue celecoxib and notify health care professional if signs and symptoms of hepatotoxicity (nausea, fatigue, lethargy, pruritus, jaundice, upper right quadrant tenderness, flu-like symptoms) occur 3.
