H.W. Heinrich
Heinrich, an insurance researcher (the guru of scientific thinking in to accident prevention) studied around 75,000 accidents in 1930s and concluded that 98% accidents are caused by unsafe acts or unsafe conditions or both together. He concluded that accidents can be prevented by eliminating unsafe conditions and unsafe acts.

He analysed 5000 accidents and concluded that on an average out of 330 accidents that are caused by same or similar conditions, there will be no injuries in 300 incidents, there will be minor injuries in 29 incidents and a disabling injury in 1 incident
