Introduction
How many times have you found lighting fixtures lying on the floor, broken into pieces, at the area where you had worked yesterday or going to work today? Worse, have you encountered, saw an object falling from height and landed without hurting anybody or damaging any equipment? Now, imagine if you were at the spot where that object landed. Most cases, Dropped Object Incidents does not result in any loss(es). However each occurrence is a DISTRACTION, and when a personnel is distracted, efficiency is downgraded as valuable time is lost discussing the incident.

From 64 incidents reported; 19 were rated as Potential LTI and 40 Potential Fatality and 5 actual LTI 57% dropped object are less than 2 kg and 24% fall from 1 meter or less 5 causes of dropped object

Preventing Dropped Objects Incidents


Lifting gear:
Ensure it is in good condition Correct size/rating Not expired Brakes are operable. Always check/test before use. Follow maintenance schedule. Maintain/preserve the lifting gears properly. Improper maintenance will lead to premature failure. Ensure correct shackles are use and installed with split pins.

Secure tools when working on elevated platform


Have a proper tool holder/container Put tarpaulin on top of scaffolding planks or gratings to prevent object slip through and drop down to the deck below.

Carry out adequate work assessment to determine the best lifting/rigging procedure.

Preventing Dropped Objects Incidents


Improve house keeping and storage:
Do not leave tool or redundant material on elevated platform. Barricade the area where overhead work took place. Send tools to where it belong. Do not mix a good and bad sling. Do not mix various sizes of shackle in a single bin. Arrange the storage properly. Do not overload the shelves.

Identify hanging equipment which have corroded bracket/support e.g. lighting, speaker, gas detector, signage, etc., and raise work order to rectify
Temporary secure with chain/clamp Ensure equipment used is compatible to work place environment/climate

Include dropped object hazards in TRA

Activity
Objective:
To educate personnel on Dropped Objects in and around the workplace

Activity should take about 30 minutes


1. Split into groups and assign to various locations/modules at your work area. 2. Look for the Dropped Object or potential of Dropped Object Incident e.g. hanging lighting fixture, unsecured materials on elevation, etc. 3. Take immediate action to rectify/eliminate hazards 4. What can you do to prevent recurrence of your findings? 5. Share your findings 6. Raise Hazards Improvement Card on any findings

Conclusion
Look out for potential of Dropped Object Incident. React to the surrounding activity. Stay clear from suspended load. Report all Dropped Object Incident, including potential incident, and learn from the findings Remember! Dropped object incident can be prevented through proper work plan and maintenance program.
