Osteoarthritis
History: 62 year old male with bilateral hip pain and decreased
range of motion. No fevers, no rashes, no significant family history of
illness.
X-Ray 1 ::
Assymetric acetabular sclerosis and joint space narrowing with
subchondral cyst formation.
Discussion:
The plain radiographic shows joint space narrowing, sclerosis, and
subchondral cyst formation. The high-pressure areas or the superior
acetabulum are most affected, suggestive of osteoarthritis (chronic
wear and tear) rather than an inflammatory arthropathy (like
rheumatoid arthritis) which would show more hemi-circumferential
joint involvment including areas that do not have heavy weightbearing responsibility.
Differential diagnosis: The differential diagnosis for subchondral
cysts is long but would include: CPPD, Osteoarthritis, Osteonecrosis,
Rheumatoid arthritis, and Synovium-based processes.
Diagnosis confirmation: Total constellation (Consens)
