OBJECTIVES FOR THIS TRAINING


By End of this training you will understanding:
Pre-Comm/Commissioning Area Access requirement.
To Indentify potential hazards and risks.
Security / Access control.
Commissioning Signage's / Tag.
Understand the emergency and evacuation procedures

Note: No workers are allowed to work on any


pre-comm or commissioning activities without
completing this training.

Why this pre-comm/commissioning training?


It is a New and Potential hazardous phase of the project.
New controls introduced to manage the hazards.
To achieve GOAL ZERO target.

What is Commissioning?
Project Commissioning is checking and testing.
To do this system will be LIVE.
LIVE system are hazardous and need careful management.
Additional control are need.

Sources of Energy
The objective of the LOTO procedure is the control of
hazardous energy sources. The procedure is used for all types
of hazardous energy sources.

What is the hazardous

Causes of hazardous energy

Isolation can be achieved using various methods


and means including:
Close valves
Bleed pressure
Deconnect
circuits
Installation of curtains/blinds
Block and purge

HOWEVER, Isolation IS NOT COMPLETE until the energy source is


locked out and marked. Materials for preventing the use of the
equipment and hence isolation of equipment/personnel from the
hazardous energy source include (but are not limited to). :

What is Lock out / Tag out?


Covers the servicing and maintenance of
machines and equipment in which the
unexpected start-up or the release of stored
energy could cause injury to employees.

What is Lock out / Tag out?


Lock
To Lock something signifies placing a padlock on
the equipment in such a manner that it cant be
energized. This is the best type of lock.

TAGS
DO NOT

TAGS are specially designed tickets attached to the switch,


valve etc. that warn the user not to energize the system. The
tag must be tied securely and be able to withstand weather
conditions such as rain or wind. It must be signed by the
authorized employee.

What is Lock out / Tag out?


Lock-out means de-energizing and putting a
physical lock with a padlock on the energy
isolating device of a piece of equipment to prevent
someone from re-energizing the equipment and
potentially causing the person servicing the
equipment to be injured.

Tag-out means de-energizing and putting


a warning tag on the energy isolating
device of a piece of equipment to prevent
someone from re-energizing the equipment
and potentially causing injury to the person
servicing the equipment.

What is Lock out / Tag out?


Tag out
Tags will not be used alone without locks and
will clearly indicate the following:
Name of the Authorized Person working on the equipment or
process
Date the lock and tag was installed

Why we use LOTO


Protect People, Equipment and the Environment.
Positive isolation of hazardous energy sources
Defines responsibilities, task and work location.
Communication, documentation and tracking.
Under multi-discipline control, Operations, Mechanical
Consistency in application

Dont touch anything
unless you have informed
your team members and
your supervisor.
Never ignore/remove tag
signed by others

When using TAGOUT please remember:


Tags are only warning devices!
Tags must be securely attached!
May evoke false sense of security!
Tags do not provide physical restraint!
Tags must never be ignored!
Must withstand environmental conditions!
Must be legible and understandable!

PTW During Commissioning


Permit for Construction and commissioning activities area different.

Both PTW are under the JVC Control.


Authorize PTW issuer for commissioning base on discipline required.

What could happen if you do


not follow LOTO?
On July 6, 1988, 167 lives
perished on the Piper
Alpha Platform, located in
the icy North Sea.

Summary

All parties understood of the safety requirement and responsibilities to meet the
overall commissioning goal of the project.
