P-Treatment

Pulmonary Tuberculosis
STEP 1: Define Patients problem
Productive cough x 4 weeks
Fever
Sweats
Weight loss
AFP Positive Smear, (+) Mantoux test positive
CXR: Apical Haziness
STEP 1: Define Patients problem

DIAGNOSIS: Pulmonary Tuberculosis


Presumptive Tuberculosis
STEP 2: Specify the therapeutic objective
To eliminate the offending agent.
Prevent development of resistance
Prevent development of relapse of disease due to dormant bacilli
STEP 3: Verify your P-Treatment
Fever: Paracetamol (Biogesic) 500 mg 1 tablet every 4-6 hours for
temperature higher than 37.5 C
SOB, Productive Cough: Ipratropium Br 20 mcg, fenoterol HBr 50 mcg
(Berodual) 10 drops in 3 mL NSS every 6 hrs and as needed.
Productive Cough: Acetylcysteine (Fluimucil) 100 mg Dissolve in Half-
glass water two times a day
Antibiotic Regimen: as listed above comply with the assigned
regimen to avoid emergence of resistance
STEP 3: Verify your P-Treatment

Sig: Put 10 drops in every 3mL NSS every 6


Sig: Take 1 tablet every 6 hours hours and as needed
for temperature higher than 37 C
STEP 4: Start the treatment

Sig: Dissolve 1 tab in half-glass


water two times a day
STEP 5: Give Information, Instructions and
Warnings
Take with or without meals. (30 minutes before meals or 2 hours after
meals)
Increase fluid intake
Follow dosage schedule and avoid drinking alcoholic beverages while
on medications
STEP 6: Give Information, Instructions and
Warnings
Instill in the patients mind the need to complete 6 months treatment
Give all medications 1 hour befire meaks. It is not ideal to breal the
dose of the drug

Follow-up monthly
