1. When Dividing Shear stopped working, the victim changed into Manual Mode in the
operation room in order to stop the machine from working.
The cause of 2. When the victim checked if there was an error of Dividing Shear cutting entry pipe or not,
the accident Dividing Shear suddenly operated.
3. We are assuming that the accident happened when the victim recheck the area after he
checked other lines.

1. Install a Dividing Shear safety cover


- Install two-way doors and proximity sensors to automatically block out electricity when
the doors are open
- Change into an available state with a restarting condition in the electricity room

Preventive 2. Install the device that stops the machine from operating automatically.
Measures in - Prevent the machine from working automatically when workers open and close protective
covers
the same - Take a measure by installing a reset button to prevent the 2 nd and the 3rd situations that
industry we cant predict

3. Record all of the situations about the Panels operation and workers by installing CCTV
in the operation room

4. Announce the cases of accidents and educate workers on precautions

changes it into Panel Mode by detecting the
disconnected state

- Though either of doors is open, electricity is automatically
blocked with the sensors. The machine will work after both
doors are closed and the condition is valid in the operation
room
- We can observe the process of the products with the naked
eye through wire netting(CCTV)

3) Installation of Device that prevents auto-restarting
- Before Installation : When the protective cover is opened and closed,
the condition of restarting is valid -> 1 Cycle test
- After Installation : Though the protective cover is opened and closed,
the machine dose not restart -> Reset button must be pushed -> 1 Cycle test
