Perioperative Nursing Phases
• Preoperative phase – begins when the decision
to have surgery is made and ends when the
client is transferred to the OR (Operating Room)
table.
• Intraoperative phase – begins when the client is
transferred to the OR table and ends when the
client is admitted to the PACU (Post Anesthesia
Care Unit).
• Postoperative phase - begins with the admission
of the client to the PACU and ends when the
healing is complete.

Assessment
Anamnesis
• Keluhan utama merupakan alasan yang menyebabkan
seorang anak dibawa oleh orangtuanya ke RS. Informasi
durasi, onset, progresivitas dan berat ringannya keluhan
utama serta keluhan dan gejala yang menyertainya .
• Riwayat penyakit sekarang dan masa lalu termasuk
riwayat operasi sebelumnya.
• Kondisi lain seperti terdapat dyspnea, riwayat sianosis,
edema, perdarahan yang sulit berhenti, dan riwayat
alergi harus ditanyakan
• Riwayat persalinan dan family tree

Preoperative Nursing Care
Physical assessment
Tujuanya adalah untuk identifikasi bagian mana yang akan
menjalani operasi dan menyakinkan bahwa sistim
organ yang lain dalam keadaan sehat
• Head to toe
• Cardiovascular system
• Respiratory system
• Renal system
• Neurological system
• Musculoskeletal system
• Nutritional status
Pemeriksaan dilakukan dengan orang tua pasien berada
di samping anak untuk menenangkannya.

• General survey- gestures and body
movements may reflect decreased energy or
weakness caused by illness.
• Cardiovascular system- alterations in cardiac
status are responsible for as many as 30% of
perioperative death.
• Respiratory system- a decline in ventilatory
function, assessed through breathing pattern
and chest excursion, may indicate a client’s
risk for respiratory complications.

• Renal system-abnormal renal function can
altered fluid and electrolyte balance and
decrease the excretion of preoperative
medications and anesthetic agents.
• Neurologic system- a client’s Loss Of Control
(LOC) will change as a result of general
anesthesia but should return to the
preoperative LOC after surgery.
• Musculoskeletal system- Deformities may
interfere with intraoperative and postoperative
positioning. Avoid positioning over an area
where the the skin shows signs of pressure over
bony prominences.
• Gastrointestinal system- alteration in function
after surgery may result in decreased or absent
bowel sound and distention.
• Head and Neck- the condition of oral mucous
membranes reveals the level of hydration.
Pemeriksaan penunjang pre op
• Tujuan: untuk mendeteksi kelainan fisiologis atau
metabolisme yang dapat meningkatkan resiko
dalam periode perioperatif
• Meliputi: foto torak, golongan darah, darah
lengkap, elektrolit darah (natrium, kalium, dan
kadar digoksin) terutama pada anak yang sedang
menjalani terapi penyakit jantung atau
mengalami gagal ginjal atau pada penggunaan
diuretik, hemoglobin terutama pada anak dengan
sickle cell, penyakit jantung, penyakit kronik atau
akan menjalani operasi yang beresiko kehilangan
banyak darah, dll.
Routine Hb is indicated
• Neonates and ex premature infants under 1 yr
• Children at risk for sickle cell ds
• Children for whom intraoperative blood
transfusion may be necessary
• Children with systemic ds
• Preoperative Hb of less than 10 g/dl is
abnormal and needs to be investigated
Pemeriksaan penunjang lainnya
Pemeriksan penunjang lain hanya dilakukan atau
indikasi seperti:
• Tes fungsi ginjal
• Tes fungsi hati
• Tes urin rutin
• Rontgen
• EKG, ekokardiografi, USG, CT scan maupun
• Magnetic Resonance Imaging (MRI)

Tes Koagulasi
Tes koagulasi dilakukan pada anak dibawah usia
1 tahun walaupun tidak ada riwayat trauma
atau perdarahan yang sulit berhenti, riwayat
prematur untuk mengetahui faktor risiko
terjadinya gangguan perdarahan seperti
rendahnya faktor IX karena hati yang imatur
dan defisiensi vitamin K.
Premedikasi
Psychological preparation
• important aspect of preoperative care,
especially for younger children
• Children aged 2-6 years are over five times
more likely to have significant preoperative
anxiety than older children and infants
• Parental anxieties concerning surgery may be
profound and can be transmitted to child .
Efforts should be made to reduce parental
anxiety as well

• Pemberian obat profilaksis
Tujuanya adalah untuk mencegah infeksi baik
selama maupun setelah operasi.
Beberapa antibiotik yang sering digunakan pada
operasi saluran urologi dan cerna:
Ampisilin+gentamisin, vankomisin+gentamisin,
amiksisilin/ampisilin.
Beberapa antibiotik yang sering digunakan pada
operasi gigi-mulut dan saluran pernafasan:
Amoksisilin, klindamisin, Sefaleksin/Sefadroksil,
Azitromisin/Klaritromisin.

Prasekolah
• Sudah memiliki kemampuan bicara yang baik, penting
untuk menggunakan kata-kata yang difahami anak
jangan yang abstrak.
• Misal penggunaan kata bius, anestesi, atau dibuat
pingsan. Gunakanlah kata kita akan membuat adek
tertidur.
• Jelaskanpada anak kenapa sakit dan harus masuk
rumah sakit (bukan karena nakal, atau membuat
kesalahan atau sebagai hukuman bagi dia)
• Berbicaralah dengan pandangan sejajar dengan anak
dan libatkan orang tua saat berbicara dengan anak.

Remaja
• fokus lebih pada anak bukan pada orang tua (dukung
kehendak dan kemandirian anak: mandi, memakai
gown)
• Ajak anak untuk mendiskusikan kasusnya dan
mengambil keputusan.
• Jagalah privasi anak
• Dukung hubungan dengan teman-temanya:
menginzinkan untuk berkomunikasi misal by
handphone menjenguk, dan menemani.
• Dukung mekanisme koping anak: mendengarkan
musik, menonton film, bermain game dll.

Complications
• Respiratory- atelectasis, pumonry Embolus
• Cardiovascular- venous thrombosis
• Gastrointestinal-Hiccoughs, N/V, abd. Distention,
paralytic ileus, stress ulcer.
• GU- urinary retention
• Hemorrhage-slipping of a ligature(suture)
• Wound infection
• Wound dehiscence and evisceration

Nursing Diagnosis
• Ineffective airway clearance- increased
secretions 2 to anesthesia, ineffective cough,
pain
• Ineffective breathing pattern- anesthetic and
drug effects, incisional pain
• Acute pain
• Urinary retention
• Risk for infection
