 Definisi IRIS
Penyakit yang tidak bergejala sebelumnya menjadi
symptomatic setelah mendapat
ART atau HAART
(“unmasking type”)

Perburukan dari suatu gejala infeksi oportunistik yang

sudah dalam pengobatan (misalnya TB, CMV, MAC)
segera setelah mulai HAART
(“paradoxical reaction”)

• Infeksi Oportunistik (IO) setelah mulai ART
(HAART) akibat pemulihan respon imun
melawan patogen
 Insidensi IRIS
• MA French et al : 25% pasien memberikan respon
baik terhadap ART, akan timbul episode IO yang
diderita sebelumnya.
• Shelburne et al: 31,7% akan timbul IO berupa
meningitis kriptokokus, TB or MAC IRIS sebagai IRIS
• Jumlah TB IRIS sekitar 1/3 of seluruh kasus IRIS
• MSF : mortalitas tinggi dalam 30 hari pengobatan
ART → IRIS ?

• Tidak sepenuhnya dimengerti

 Hipotesis
• IRIS: successful reconstitution of
effector/memory T cells but regulatory T
cells (Tregs) still deficient?
– Tregs are the major player in the prevention of
immunopathological events (IBD, helminth
diseases, autoimmune diseases etc)
– Simple markers of Tregs not (yet) available

• Early IRIS (dalam 3 bln pertama setelah ART)
– Karena respon imun terhadap patogen aktif yang
sering timbul dalam infeksi sub klinis

• Late IRIS (bulan-tahun setelah ART)

– Karena respon imun terhadap antigen oportunistik
yang tidak aktif
 Presentasi klinis

Gambaran klinis IO yang berhubungan dengan

IRIS dapat :
- berbeda dari gejala IO pada penderita dengan
immune compromised (mis. MAC) atau
- Identik/sama dengan penderita
immune compromised (mis. H. Zoster)
 TB - IRIS
• Demam
• Limfadenopati perifer, abses
• Thoraks foto : Perburukan infiltrat
• CT scan : tuberkuloma intrakranial
• Fatal
• BTA dan kultur mungkin (-)
• Timbul 1-6 minggu pertama ART

 Penatalaksanaan TB - IRIS
• ART teruskan
– Bila berat → hentikan
– Subsitusi regimen bila perlu (NVP → EFV)
• Mulai pengobatan TB (OAT)
• Steroids* bila diperlukan
– Misalnya pada kasus berat : Meningitis TB, sesak berat

* note: rifampicin induces steroid metabolism

 Meningitis Kriptokokus - IRIS
• Sakit kepala timbul lagi
• Meningismus
• LCS : peningkatan tekanan intra kranial
– Lekosit meningkat
– Meningitis aseptik
• Kultur kripto sering (-)
• Terjadi pada 1-2 bulan (lebih dari 4 hari hingga
15 bulan)
IRIS and cryptococcal meningitis: definitions

Classical type:
1. condition had clinically improved with anti-crypto Rx
2. after intro HAART: return of initial symptoms or new
inflammatory symptoms
3. all cultures neg, and CSF Ag if still present: should be 4x less
than initially
Unmasking type:
1. new onset of symptoms < 180 days after starting and
responding to HAART
2. CSF cultures neg
3. blood CRAG pos
4. CSF CRAG pos with titer < 1:8
5. CSF WBC > 50 cells/microliter
Penatalaksanaan M.Kripto - IRIS
• HAART teruskan (perlu penundaan ART,
setelah terapi ?)
• Terapi ulang antifungal. Tetapi bila pasien
masih dalam terapi fluconazole 400 mg,
tidak perlu Ampho B.
• Hindari start HAART < 30 hari setelah
diagnosis + pengobatan kriptokokus
 Herpes Zoster - IRIS
• Gejala klasik/tipikal herpes zoster
• Terjadi 1-8 bulan (lebih 80 weeks)
• Terapi :
– HAART teruskan
– Acyclovir pada kasus lesi nekrotik, luas atau pada
ophtalmic zooster
• Hati-hati efek samping obat
 Hepatitis virus - IRIS

• Sulit dibedakan dengan keadaan hepatotoksik

• Hepatitis B atau C
• Pada bulan ke 1-9
• Terapi : ART teruskan
 IRIS and CMV
• Ocular: retinitis, uveitis, vitritis
• Extra ocular : kulit, kolitis
• Tidak selalu mempunyai riwayat CMV
• Terjadi dalam 1-2 bulan setelah ART
• Terapi :
– ART teruskan
– Gansiklovir (intravitreal)
– Steroid ?

• Perbaikan antigen spesifik limfosit T secara

kualitatif dan kuantitatif (JC virus)
• Perburukan gejala klinis → sering dengan ↗

• Kadang-kadang fatal
• Terapi : steroid mungkin bermanfaat
 IRIS and MAC (Mycobacterium
Avium Complex)
• Demam
• Limfadenopati atau limfadenitis
• Abses (kulit, abdomen)
• Infiltrat pada paru
• Kultur lymph node : sering (+)
• Terjadi 1-52 minggu setelah ART
 MAC - IRIS
• Terapi :
– ART teruskan
– MAC : claritromisin (azitromisin), ethambutol dan
levofloksasin
– Steroid
– Drainase bila diperlukan
 Other examples of IRIS
• Herpes Simplex (skin or encephalitis) – cases
described that were not responsive to regular
therapy nor steroids (4 weeks – 6 months after
HAART)
• Kaposi sarcoma: flares of existing lesions, or
appearance of lesions in new sites
• Molluscum contagiosum
• Histoplasma Capsulatum (lymph nodes)

• Graves disease (hyperthyroidism with thyroid Ab)
• Rheumatoid Arthritis
• Leprosy
• Guillain-Barré
• Toxoplasma

• Reactions on old tattoos, tribal marks

 Other examples of IRIS (3)
• Sarcoidosis
• Leishmaniosis
• Several dermatological conditions
(eosinophilic folliculitis, condyloma, warts…)
• PCP  do not start HAART within 18 days
after start of PCP therapy
• Strongyloides stercoralis
• Vulval ulcerations
 Diagnosis banding IRIS

• Toksisitas ART seperti NVP

• Gagal pengobatan IO (MDR – TB)
• Gagal pengobatan ART
 Kesimpulan
• Definisi IRIS sulit ditentukan
• Proses yang mendasari salah satunya adalah
respon inflamasi yang bertambah setelah ART,
menimbulkan perburukan klinis meskipun
dengan perbaikan VL dan CD4
• Jika IRIS tidak dapat didiagnosis, baiknya ART
dihentikan karena gejala IRIS ≈ drug toxicity
(hepatitis)
 Kesimpulan
• IRIS mungkin dapat diinterpretasikan sebagai
kegagalan terapi (seperti pada meningitis
kriptokokus)
• Pengobatan :
– ART teruskan bila memungkinkan
– Terapi IO
– Steroid → belum ada panduan yang jelas
