Functions of Blood
Blood performs a number of functions dealing with:
Substance distribution Regulation of blood levels of particular substances Body protection
Distribution
Blood transports:
Oxygen from the lungs and nutrients from the digestive tract Metabolic wastes from cells to the lungs and kidneys for elimination Hormones from endocrine glands to target organs
Regulation
Blood maintains:
Appropriate body temperature by absorbing and distributing heat Normal pH in body tissues using buffer systems Adequate fluid volume in the circulatory system
Protection
Blood prevents blood loss by:
Activating plasma proteins and platelets Initiating clot formation when a vessel is broken
Composition of Blood
Blood is the bodys only fluid tissue It is composed of liquid plasma and formed elements Formed elements include:
Erythrocytes, or red blood cells (RBCs) Leukocytes, or white blood cells (WBCs) Platelets
Plasma (55% of whole blood) Buffy coat: leukocyctes and platelets (<1% of whole blood)

Blood Plasma
Blood plasma contains over 100 solutes, including:
Proteins : albumin, globulins, clotting proteins, and others Nonprotein nitrogenous substances : lactic acid, urea, creatinine Organic nutrients : glucose, carbohydrates, amino acids Electrolytes : sodium, potassium, calcium, chloride, bicarbonate Respiratory gases : oxygen and carbon dioxide
Protein plasma
Fungsi umum : Membentuk tekanan koloid osmotik untuk mencegah cairan plasma keluar pembuluh. Menentukan viskositas darah Menyangga perubahan pH Dalam kondisi kelaparan dimanfaatkan sebagai sumber energi.
Fungsi khusus : Albumin : mengikat banyak zat untuk ditransportasi (bilirubin, garam empedu, penisilin), sangat berperan menentukan tek.koloid osmotik (karena jumlahnya).
Globulin : tiga subklas Alfa & beta untuk mengikat & mengangkut zat (hormon tiroid, kolesterol & besi). Sebagai faktor yang berperan pembekuan darah Alfa : sebagai protein prekursor inaktif (cth : angiotensinogen) Globulin gamma : imunoglobulin untuk pertahanan tubuh.
Fibrinogen : faktor dalam proses pembekuan darah. Protein plasma disintesis oleh hati, kecuali globulin gamma yang dihasilkan limfosit.
Formed Elements
Erythrocytes, leukocytes, and platelets make up the formed elements Only WBCs are complete cells RBCs have no nuclei or organelles, and platelets are just cell fragments
Erythrocytes (RBCs)
Biconcave discs, anucleate, essentially no organelles Filled with hemoglobin (Hb), a protein that functions in gas transport Contain the plasma membrane protein spectrin and other proteins that:
Give erythrocytes their flexibility Allow them to change shape as necessary
Structural characteristics contribute to its gas transport function Biconcave shape that has a huge surface area relative to volume Discounting water content, erythrocytes are more than 97% hemoglobin ATP is generated anaerobically, so the erythrocytes do not consume the oxygen they transport

Hemoglobin reversibly binds with oxygen and most oxygen in the blood is bound to hemoglobin
Eritrosit mengandung carbonic anhidrase yaitu enzym yang mengkatalisir reaksi bolak balik H2CO3 H2O + CO2 (lungs) H2CO3 H2O + CO2 (tissue) Dengan seringnya terjadi reaksi ini maka CO2 akan semakin cepat dikeluarkan dari tubuh, dan ph darah tetap stabil
Eritrosit juga mengandung glycolytic enzym yang menghasilkan bahan bakar untuk mekanisme transport aktif Eritrosit merupakan buffer asam basa yang baik karena mengandung molekul Hb yang sebagian besar pembentuknya adalah protein Hb + H+ HHb
Hemoglobin is composed of the protein globin, made up of two alpha and two beta chains, each bound to a heme group
Each heme group bears an atom of iron, which can bind to one oxygen molecule Each hemoglobin molecule can transport four molecules of oxygen

Erythropoiesis
Erythrocytes survive for 100-120 days in circulation. Remnants broken down by liver and spleen. Renewed by division and differentiation of stem cells found in the bone marrow: 2 million/second! Formation controlled in part by the hormone erythropoietin (EPO).

Regulation of Erythropoiesis
Pada orang yang hidup di lingkungan dengan kadar O2 yang rendah ,eritropoetin tetap banyak disekresikan - meskipun jumlah sel darah merahnya normal. Hal ini terjadi karena ginjal menangkap sinyal hipoksia jaringan akibat rendahnya tekanan oksigen Oleh karena itu orang yang hidup di pegunungan jumlah eritrositnya lebih banyak dari nilai normal
PEMATANGAN SEL DARAH MERAH DAN KEBUTUHAN AKAN VIT B12 DAN AS.FOLAT
Proses pematangan dan jumlah eritrosit yang dihasilkan sangat bergantung pada status gizi seseorang pada proses terakhir pematangan, vit B12 dan as.folat sangat dibutuhkan untuk pembentukan thymidine tryphosphate yang merupakan bahan penting pembentuk DNA Kekurangan kedua zat ini akan menyebabkan terbentuk DNA yang abnormal Dan kegagalan pematangan nukleus atau bagian sel lain terbentuk sel darah merah yang ukurannya lebih besar, membran yang tipis, rapuh, irreguler dan berbentuk oval (makrosit)
Kekurangan vit B12 biasanya disebabkan kurangnya asupan makanan yang mengandung zat tersebut dan malabsorbsi di sal. Pencernaan Malabsorbsi biasa disebabkan oleh atrofi mukosa lambung sehingga parietal sel tidak dapat mensekresikan intrinsic factor yang seharusnya berikatan dengan vit B12 dalam makanan sehingga dapat diserap dengan baik di ileum and kemudian masuk ke dalam aliran darah
Vit B12 disimpan didalam hati,dan akan dilepaskan sesuai kebutuhan untuk memproduksi sel darah Kebutuhan minimum vit B12 yang diperlukan untuk produksi sel darah merah adalah 1-3 m/hr sedangkan yang disimpan di hati dan jaringan lain pada orang normal adalah +/- 1000x dari jumlah ini Sehingga dibutuhkan 3-4 th masa kekurangan vitB12 untuk dapat menimbulkan anemia pernisiosa
